HCCMS Parent Input for Team Placement


The Parent Input link for the 2018-19 school year will be made available mid-May. The link will be posted on hccmsonline.com. It will
remain open for one week. Your input is important to us and will be considered, but not guaranteed. Being on the same team does not
mean being in the same classes. Academic needs, state/federal requirements and class count rules prevail. Please use the comment box on
the input form to share how your child performs best. The input form is a great tool for communicating how to set your child up for
success!

Georgia’s 7th Grade Immunization


Requirements

Georgia’s immunization requirements for children attending 7th grade have been revised.

Effective July 1, 2014 children born on or after January 1, 2002 who are attending 7th grade and for new entrants into a Geor-
gia school grades through 12th must have received one dose of Tdap (tetanus, diphtheria, pertussis) vaccine and one dose of
meningococcal conjugate vaccine.

“New Entrant” means any child entering any school in Georgia for the first time or entering after having been absent from a
Georgia school for more than twelve months or one school year.
Immunization forms are being accepted NOW at HCCMS!!! Beat the rush and get yours in early!

The GA Public Health Certificate of Immunization (Form 3231) can be mailed or faxed to:

DUE May 18th!!! To get your team assignment and schedule in a timely manner, meet the
State’s 7th grade health requirement.
HCCMS 2018-19
Softball Season Information

HCCMS will field a Tiger Softball Team fall 2018-19 school year!
 Tryouts will be held May 21-24 from 3:00-5:30 PM at the HCHS softball field. Students must attend at
least 2 of the 4 tryout dates listed. Skills will
 Students MUST be in grades 6th-8th for the 2018-19 school year. 8th graders will have an option to
play for HCCMS or the Harris County High School’s junior varsity team .
 Students MUST have a current physical, medical release and concussion form on file with the 7th
grade office before being allowed to tryout. Physicals are due by May 4th.
 There will be a bus available to take students to the high school field from HCCMS and Creekside to
Harris County High School (HCHS).
 Parents pick up students at HCHS softball field at 5:30 PM.

 HCCMS baseball will be offered in the spring of the 2018-2019 school year.

HCCMS Sports Physicals- HCCMS will be offering onsite athletic physicals April 26th starting at 3:00 p.m. If
you would like for your child to stay after school April 26th and receive a physical, please sign up in the
7th grade office. This is first come first served.
To have your child's name on the list:
 parents must complete the history portion of the physical form completed by April 23rd.
 send a payment of $10 (cash) by April 23rd
 parents should be prepared to pick up their child at HCCMS by 4:30pm in the front lobby.

Physical forms can be found on our website at hccmsonline.com under the sports tab or in the 7th grade
office. If you have any questions please contact Mrs. Clark at 706-628-4951 or clark-l@harris.k12.ga.us.
Payment and forms must be turned in to the 7th grade office by April 23 to be pre-approved, have trans-
portation arranged from Creekside to HCCMS and have an appointment reservation .
