SCOPE OF WORK UNDER SERVICE CONSULTANCY AGREEMENT

Self-service Basic Laundry Business Set-up

A. General Inclusions:

2.4 Dryer Exhaust System PVC piping of the exhaust system for the dryer.
2.5 Electrical Wiring System Electrical wiring system for the washer and dryer.

3. Pressure Tank & Motor Installation

3.1 Storage Tank 125 gallons
3.2 Motor Pump & Bladder Pressure Tank 19 Gallons

4. Post Installation Support

4.1 One (1) year warranty on parts & services of Washer & Dryer.
4.2 One (1) year Quarterly maintenance of Washer & Dryer.
4.3 One (1) year Quarterly cleaning of ductwork.
4.4 Three (3) months warranty on workmanship.
4.5 Continuous Operational & Technical Support.

B. Project Team Composition

The total contract fee includes hardware materials, manpower labor, training on laundry operation, training on proper usage of machines, and
professional fee of consultants and supervisor.

E. Terms of Payment
1. For Basic Renovation & Set-up & Installation of Equipment is amounting of Php 175,000.00.
50% down payment upon signing of contract 87,500.00
40% prior to the delivery of machines 70,000.00
10% upon completion and turnover 17,500.00

2. For 5 units LG Washer and 5 units Dryer Machines with Centralized Card System amounting to Php 925,000.00.
50% machine down payment 462,500.00
50% prior to the delivery of machines 462,500.00

F. Schedule for Payment

 G. Other Conditions:
· Basic set up Completion: 20-25 working days for Visayas and Mindanao Client.
· Delivery of Machines is within 3 to 5 working days after full payment within Luzon Area except via Shipping Cargo.
· Delivery of Machines is within 10 to 20 working days after full payment via Shipping Cargo.
· Delivery of Machines: free within Metro Manila.
· Additional Charges for Provincial Client.
· Turnover: Five (5) working days upon delivery of machines.
· Set-Up and Installation for Mindanao and Visayas Area will start after delivery of machines.
· Fare, lodging of One (1) Account Executive for Ocular Inspection, Two (2) Skilled Personnels, Two (2) Machine Technicians must
be shoulder by the client for Provincial area.
· For Clients preferring to pick up the machines from Kyzen office personally or thru their own cargo service, negligence,
mishandling, and other Acts of God wil not be covered by warranty.
· All above remains the property of KYZEN Business Consultancy., until fully paid.

· Business Service set up & Consultancy fee is EXCLUSIVE OF VAT

· LPG & Gas Tank is not included in this package.
· Package is applicable on shop locations that have ceilings, floor tiles glass door and walls ready for painting.
