Scheduling an Appointment
● All appointments made in person/by phone
● Brief Screening Appointment (initial visit)
○ includes questionnaire and surveys, 45 minutes
○ in-person visit, until 4 pm
● Canceling an Appointment
○ Call or cancel online at least 24 hours prior to appointmetn
○ $20 fee for late cancellation/missed appointment
■ $50 fee for limited eligibility students
