Source of Evidence 10: Collaboration

GUIDELINES
1. Meet with your professor or cooperating teacher to establish a collaboration project
and to target a timeline for completing it (approximately a two-week time frame).
2. Use information from context and from your professor/cooperating teachers to identify
a student whose learning would be enhanced by collaborative efforts. The student
could have needs because of learning challenges (e.g., students with IEPS, 504 plans,
or LEP students), students whose needs are a result of their strengths (e.g. gifted
students), or students whose engagement is limited (e.g., over-commitment to part-
time jobs, family responsibilities, or extracurricular activities). (does not need to be a
special ed student b/c 9 is the year of identifying_)
3. Complete this form.
4. Report your progress in developing and implementing your plan to your University
Supervisor.

Note to all students: If the analysis of assessment data reveals that the collaboration plan
had little or no impact on student learning, you have the opportunity to reflect on the plan
and identify possible alternative approaches that could be used in the future.

a. Objectives – what you plan to accomplish (in terms of student learning):

Students will be able to Identify and comprehend the chords C, F, and G7 on


the ukulele as well as perform them by using the song Twinkle Twinkle Little
Star.

b. Assessment plan – how you will measure the impact of the collaborative effort
on student learning?

I will measure the impact by the student’s ability to perform Twinkle Twinkle Little
Star and it’s fluidity as well as assessing how well the student plays the chords.

2. Using the assessment data collected, describe the impact of the collaboration
plan on this student’s learning and describe possible next steps.

The student is still having trouble with the G7 chord, and remembering the fingerings
of each chord. She is able to play the C chord and F chord when I lay her fingers on the
board, but I believe that she needs more work and with more practice or even letting
her practice on her own time at home she will be able to play Twinkle Twinkle Little
Star with no stops in between chords.
