FREQUENCY (UHF) RADIO-FREQUENCY
IDENTIFICATION(RFID) CARD

Greetings!
Considering the great volume of students, employees and stakeholders
entering the SLU campuses, an added feature of fortifying campus safety and
ensuring stricter security protocols is set to be adopted.
Relative to this, and in line with the University’s earnest efforts of
streamlining its processes to better serve you, please be informed that
beginning this Short Term 2018, all students will be using the UHF RFID card
in transacting with the University.
In preparation for this transition and the logistics accompanying it,
students who are to complete their current degree beyond this semester shall
personally apply for their identification card by accomplishing the Perpetual
UHF RFID Card Processing Form available in your respective Dean’s Office.
Graduating students this Second Semester 2017 - 2018 are exempted from this
application.
Students, who will not anymore enroll in the coming terms for certain
reasons (e.g., transfer to another school, migration to another country),
should sign a notification form available at the school Dean’s office and
submit said notice to the Finance Office.The accomplished form for this transition will then be presented during
the scheduled date required picture-taking in specified venues as follows:

The cost, Php 300.00, which comes with the new identification card,
shall be reflected in the assessment of fees for the final examinations. Your
UHF RFID card, which will be activated upon your next term enrolment, will
be claimed from the Office of the Registrar upon presentation of your claim
slip and official SLU receipt of your Short Term 2018 or First Semester AY
2018-2019 enrolment.
