This document contains a standardized assessment tool called the Child and Adolescent Needs and Strengths (CANS) used to evaluate the needs and strengths of a child or adolescent across multiple domains of functioning. Various life domains, caregiver resources, child strengths and needs, risk behaviors, and cultural factors are rated on a 4-point scale from no evidence of need to disabling or dangerous need. The assessment also includes optional modules that can be completed if indicated. The full CANS is used to develop individualized service plans for children and monitor outcomes over time.
