DECLARATION BY THE CANDIDATE:

I hereby solemnly declare that:

a) the information given in this Admission Form is correct to the best of my knowledge and belief and if
anything is found incorrect, the University will have the right to cancel my admission and expel me
from the University.
b) I promise to:
i. abide by the Statutes, Regulations and Rules etc. framed by the University from time to time;
ii. maintain good behavior;
iii. work diligently and maintain the dignity and prestige of the University both on and off the
Campus.
c) I undertake to be a full-time and regular student of the University and shall not join any other
Programme or accept any employment for the duration of the Programme registered. (The in-service
teachers/persons are required to produce NOC/Study Leave duly sanctioned by the employers).
d) I further undertake that I will not claim hostel accommodation as a matter or right if admitted in the
University.
e) I promise to pay all the dues regularly.
f) I hereby declare as binding on me, that as long as I am a student, I accept all the Statutes, Regulations,
and Rules in force at the time of joining the University and framed subsequently. I shall submit to the
discipline of the University authorities as exercised through its various Committee and Officers.
g) I accept as a condition of my Admission the authority of the University to the effect that if in the
opinion of the Vice-Chancellor my stay is not conducive to the normal academic and community life
on the campus, I will not hesitate to withdraw my name from the University after being called upon
me to do so, and my admission will be treated as cancelled.

INSTRUCTIONS FOR THE CANDIDATES

1. All entries in the form must be made in BLOCK LETTERS or must be typed.
2. Strike out what is not applicable, but do not leave any entry unfilled.
3. In case an entry does not apply to you, clearly write NOT APPLICABLE. In all other columns write the
necessary information or write appropriate replies, such as yes, no, nil etc.
4. Three (03) attested copies of the following documents should accompany the applications:
a. All the degrees and certificates of education mentioned on the first page of this form.
b. All the detailed marks certificates of education mentioned on the first page of this form.
c. Equivalence certificate from Higher Education Commission (HEC) in case of ‘O-Level’, ‘A-Level’ or
other qualifications from foreign Universities/Institutes.
d. Domicile certificate of the candidate.
e. Character Certificate from the Institution last attended.
f. Migration Certificate (in original).
g. No Objection Certificate (in case of in-service candidate).
h. National Identity Card of the Candidate.
i. National Identity Card of the Candidate’s Father/Parent/Guardian.

NOTES:

1. The selected candidates will be required to produce originals of all the above documents.
2. The selected candidates will be required to submit an affidavit on a stamp paper to the effect that
the certificates submitted are genuine and in case there are found fake/bogus/tampered, the
University will have the right to cancel the admission and initiate legal action. The candidate will
also be required to produce an affidavit in accordance with the decision of the Supreme Court.
3. Candidates having obtained certificates/degrees from foreign universities/institutions are required
to provide equivalence certificates and the conversion of grades/CGPA into marks for all the
certificates from Inter Board Committee of Chairmen.
4. Candidates who are more than 22 years of age on the closing date of admission are not eligible for
admission.
5. Foreign candidates should route their applications through the Ministry of Education, Government
of Pakistan,
6. In case of incorrect/misleading information or concealment of facts, the University will have the
right to refuse/cancel the admission and expel the student from the University.
7. Incomplete applications will not be entertained.
