JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, MERCEDES A. BADAJOS and REBECCA S. VILLAMOR, both of legal

ages, Filipinos, and both residents of Poblacion, Lantapan, Bukidnon, after having
been duly sworn to in accordance with law, do hereby depose and state:

1. That we personally know RAFILA JAVINES BINANLAO who was born on April 8,
1958, at Lantapan, Bukidnon;

2. That RAFILA JAVINES BINANLAO grew up with us in the community and we are
aware that she has been using the name RAFILA JAVINES BINANLAO in all her
private and public transactions;

3. That however, it was recently discovered that the name entered in her
Certificate of Marriage was RAFELA JAVINES BINANLAO, instead of RAFILA
JAVINES BINANLAO, her true and correct name;

4. That RAFELA JAVINES BINANLAO and RAFILA JAVINES BINANLAO refer to one
and the same person;

6. That this is executed to attest to the truth of the foregoing facts and to support
for whatever legal intents and purposes this may serve.
