MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This MEMORANDUM OF AGREEMENT is made and executed this

______________ in _______________ by and between:

Dalan Sa Kalambuan Foundation, Inc. (DALSAKA), a non-stock, non-

profit institution, duly organized and existing under Philippine Laws with
office/business address at MSU, Tambler, General Santos City, Philippines represented
herein by SYLVIA R. OCTURA, DALASAKA BOT Chairman, hereinafter called the
DONOR FOUNDATION;

-And-

__________________________, of legal age, Filipino, married and

resident of _________________________________________, General Santos City,
Philippines, hereinafter called the BENIFICIARY.

-WITNESSETH-

1. WHEREAS, the FIRST PARTY is the representative/Chairman of DALASAKA

Foundation, Inc., having a project called ARISE Mindanao which was started to help
and provide capacity building programs towards sustainable livelihood in specific
community where the second party situated;
2. WHEREAS, the SECOND PARTY is a beneficiary of the ARISE Mindanao
project which is the Home Livelihood. The said project is to give a capital to the said
family beneficiary for Raw Tourism: two (2) cottages with water system and to
develop a sustainable Small Scale Tourism in the identified area. The amount given is
P35,000.00;
3. WHEREAS, BOTH PARTIES have entered into an agreement to be good
stewards of the amount given, to faithfully work until the capital will gain profit and to
help the family to have sustainable income to start on their own.
4. WHEREAS, BOTH PARTIES have agreed that the said capital will be
transferred to the next family beneficiary at the choice of the previous beneficiary with
the approval of the donor foundation. The said transfer of capital to another beneficiary
will have the same agreement to be observed thereof;

 4. WHEREAS, BOTH PARTIES have also agreed to terminate this agreement
until the seed capital will be transferred to the next family beneficiary, and purpose of
the donation will be satisfied.
