New Dumangas National High School (NewDNaHS) enrolled JESAMAY BEN

NALON dated June 5, 2017 for the School Year 2017-2018 as a regular Grade

Seven (7) student, Section Anthurium under the advisory of Ms. Lora May Bebelone

Laurella.

Given the supporting documents such as Live Birth Certificate, Form 138-E

(School Report Card-Grade 6), Form 137-E (Elementary Permanent Record) and the

screenshot of LIS data, Jesamay was admitted as a regular student of New

Dumangas National High School.

Upon checking and validation of records by Ms. Laurella she noticed that the

age of Jesamay is 17 years old, with September 30, 2000 as her birth date. This

data doesn’t fit with her present grade level considering that she is a regular enrolled

student. Jesamay was asked by her adviser (Ms. Laurella) of her exact birthdate,

she told Ms. Laurella that her birthdate is January 30, 2005. Supported with the

statement of her classmate, Kayla Flang Tuan, the real name of Jesamay is JOLINA

KUYAN NALON. Relative to this findings, Ms. Laurella referred the scenario to the

LIS Coordinator of NewDNaHS, Mrs. Cherry May Tusalem Arellano and in the

presence of the School Principal, Mr. Nestor Andrion Raguindin.

Mrs. Arellano checked the records of Jesamay and compared these records

within the Learners’ Information System of the Department of Education (DepEd).

She found out the incongruity of data. She suggested to the school Principal to call

the attention of the mother of the child for further investigation and more inquiries.

Dated October 26, 2017, Thursday, MRS. FENING KUYAN NALON appeared

in the office of the School Principal for some inquiries about her daughter’s identity,

of course, with the presence of Mrs. Arellano – LIS Coordinator and Ms. Laurella –

Class Adviser. Mrs. Nalon was asked for the real name of her daughter; she told it

was JOLINA KUYAN NALON. Moreover, she was also asked, if who is JESAMAY

BEN NALON, she told that Jesamay is the daughter of MRS. MELANIE BEN

NALON, (deceased) other wife of her husband JAIME GAL NALON.

After such inquiry and information gathering, NewDNaHS forwarded written

request to the South Cotabato DepEd Division Office, dated November 22, 2017,

requesting the said office to check the enrolment history of JESAMAY B. NALON

within the LIS.
