Notes:
For Courses in the existing curriculum – the HEI should fill up rows with courses included in the most recent revision of the curriculum
duly noted by CHED MIMAROPA. The HEIS should include a copy of this curriculum in the attachments to this form.
For Minimum Requirements based on current PSGs – the HEI would fill up rows with the minimum course requirements stipulated in the
Policies, Standards and Guidelines.
For the Courses in the proposed curriculum – the HEI should fill up the rows with courses to be offered that complies with the minimum
requirements. The HEIs should include a copy of the proposed curriculum as attachment to this form.
The school can add courses beyond the minimum provided that these courses support the HEIs vision, mission and objectives and the
courses addressed specific outcomes identified.
