Dear Ms. Roberts,

Re: Mrs. Jasmine Thompson, aged 75

Mrs. Thompson who has been suffering from right shoulder osteoarthritis and has undergone
TSR, is being referred to you for continuation of care and home assistance. She is being
discharged today to their home.

Postoperatively, she responded well to the treatment and attained a good recovery. She has
been commenced on physiotherapy and has to wear a shoulder brace for four hours per day.
For postoperative pain, analgesia and application of cold compress on her right shoulder were
also started.

A right arm sling should be maintained and she will not be allowed to lift for four weeks. Her
current medications include Voltaren and Panadeine Forte every 6 hours for pain.

It would be greatly appreciated if you could administer Clexane, to be given subcutaneously for
four days. This will avoid deep vein thrombosis postoperatively. Your assistance is also required
when she is taking a shower. Kindly note, that her shoulder staples will be removed on her
follow-up appointment, 10 days postoperatively. Should you require any
