MARRIAGE
REQUIREMENTS
1. At least two (2) valid IDs of applying couple upon personal appearance (strictly required)
2. Valid ID of parents upon personal appearance (strictly required) and accomplished Parental
Consent (18-21 years old) or Parental Advise (22-25 years old) duly verified and taken oath in front of the
Civil Registrar
3. Birth Certificate (new) – 1 original & 2 photocopies
4. Certificate of Attendance to Pre-Marriage Seminar by CHD – 1 original & 2 photocopies
5. Barangay Certificate – 1 original & 2 photocopies
6. CENOMAR (Certificate of No Marriage issued by NSO) – 1 original & 2 photocopies

Death Certificate of deceased spouse issued

by Local Civil Registrar or NSO
1. Accomplished Certificate of Live Birth (COLB) in four (4) copies, original signatures affixed;
2. Backside of COLB (Affidavit for Delayed Registration of Birth) must be accomplished and
notarized or subscribed by authorized Officer;
3. Certificate of No Record issued by Civil Registrar;
4. Negative Certification issued by National Statistics Office;
5. Any two of the following documentary evidences, showing Name of Child, Name of Mother and
Name of Father, to wit:
6. If illegitimate during birth of child, and the mother is not the applicant/informant attached;
7. Affidavit of two disinterested persons attesting to the circumstances of the birth of the child

2. Certificate of No Record issued by Civil Registrar;
3. Negative Certification issued by National Statistics Office;
4. A copy of the Marriage Contract obtained by the couple from the marriage ceremony with License
or Affidavit of Art. 34 – 1 original & 2 photocopies
5. Affidavit executed by the Church/Solemnizing Officer OR by the couple attesting to the existence
of the marriage ceremony and the reason for the delayed registration – 1 original & 2 photocopies;
6. Payment/Fee:
1. Accomplished Certificate of Death (COD) in four (4) copies, original signatures affixed;
2. Backside of COD (Affidavit for Delayed Registration of Death) must be accomplished and
notarized or subscribed by authorizing officer;
3. Certificate of No Record issued by Civil Registrar;
4. Negative Certification issued by National Statistics Office;
5. Death Certificate issued from the Church;
6. Affidavit executed by the Hospital/Clinic Administrator OR attendant at death if death occurred in
the said institution of under the care of the attendant; in default of the afore-mentioned the Affidavit shall be
executed by the default, shall be executed by the nearest relative or the person having charge of the patient;

REQUIREMENTS
1. Three (3) Original Copies (with original signature) of the Affidavit to Use the Surname of the
Father (AUSF), typewritten and duly signed by the father;
2. Three (3) Original Copies of Affidavit of Consent of the Mother;
3. Two (2) photocopies of birth certificate (Form 1A) or Certificate of Live Birth of the child;
4. Any document showing clearly the paternity between the father and the child: (Strictly required
when the name of the father does not appear in the birth record of the child), i.e.;
a. Employment records (with benefeciaries)
b. SSS/GSIS records
c. Insurance
d. Statement of Assets and Liabilities
e. Individual Income Tax Return (ITR) of the father
f. Others (showing relationship between father and child)
1. Three (3) original copies of Affidavit of Acknowledgement/Recognition duly signed by the father
(must be typewritten/computer printout);
2. One (1) original and two (2) photocopies of Birth Certificate (Form 1A) or Certificate of Live
Birth;
3. Any document showing clearly the paternity between the father and the child, i.e.;
a. Employment records (with benefeciaries)
b. SSS/GSIS records
c. Insurance
d. Statement of Assets and Liabilities
e. Individual Income Tax Return (ITR) of the father
4. Show valid ID of the parents (i.e. SSS/GSIS ID, Voter’s ID, Postal ID, Driver’s License, Passport,
etc.)
1. Three (3) original copies of Affidavit of Legitimation duly signed by the parents (must be
typewritten/computer printout);
2. Three (3) original copies of Affidavit of Recognition by the father (if the father’s name does not
appear in the original birth certificate);
3. One (1) original and two (2) photocopies of the Marriage Contract of the parents;
4. Certificate of No Marriage CENOMAR, from the National Statistics Office of both parents;
5. Any document showing clearly the paternity between the father and the child, i.e.;
a. Employment records (with benefeciaries)
b. SSS/GSIS records
c. Insurance
d. Statement of Assets and Liabilities
e. Individual Income Tax Return (ITR) of the father
