INCIDENT REPORT

Good am, Sir/Ma’am this is to inform you that last April 14 around 1:30pm in the afternoon I instructed
our Leadman on painting works Mr. Eduardo Ernacio Sr. (Leadman) to finish the painting works for Court
Floor Repainting Project that must be done on April 15 (Sunday). Both of us agreed that he will finish
this job on Sunday w/ the help of three other painters current deployed at Dorm 2 Repainting Project.
This schedule is well coordinated to Ms. Mary Gel Soldevilla of SMIP as I mentioned to her last Saturday
regarding our agenda. Unfortunately the painting works that we are expecting to finish w/in the day of
April 15 didn’t push through due to the reason that Mr. Eduardo Ernacio Sr. failed to attend the
extended work schedule.

On April 16 in the morning I instructed Leadman Danilo Bana to finish this job w/ in the day, I only failed
to inform Ms. Mary Gel regarding this matter that results in conflict w/ the team from PBI that will used
this court on that day.
