 Patient Safety: Prevention of harm to patients [1]

 Quality of Care: Health services for individuals and populations increase the
likelihood of desired health outcomes and are consistent with current
professional knowledge [1]
 Medical Errors were first highlighted in 1999 by the Institute of Medicine (IOM)
 To Err is Human: building a Safer Health System [2]

 44,000-98,000 people die annually from medical errors


 8th leading cause of death in United States
 Costs approximately 17 billion dollars annually

 Latent: affects organizational policies, procedure, and resources


 Active: directly affects patient
 Organizational: indirect failure in regards to management, protocols, and
external factors
 Technical: indirect failure of facilities

 IOM initiated a movement focused on improving the Nation's quality of care [3]

 1999-2001: establishing a vision and plan
 Phase 3
 2001- Present: ongoing focus to achieve vision

Significance to the Nursing Profession
 Keeping Patients Safe: Placed in all health care organizations for Nurses to
