resident of Carmen, Pandan, Antique do hereby name, constitute
and appoint NILDA L. AMBAY with postal address at Brgy.
Carmen, Pandan, Antique, Philippines as my true and legal
representative to act for and in my name and stead and to
represent me, to do any or all of the following acts, to wit:

1. To transact with the Department of Education Region VI for

the purpose of authenticating my following documents:
Certificate of Good Morale Character, Form 137, and
Diploma.

2. To sign, execute, and deliver documents that may be required

by the said institution/s in connection with the said
authentication;

3. To receive documents pertaining to the said authentication;

HEREBY GIVING AND GRANTING unto my said ATTORNEY-

IN-FACT full power and authority to do and perform all and every
act and deed whatsoever requisite and necessary to be done in and
about the premises, as fully as to all intents and purposes as I may
or could lawfully do if personally present with all the power of
substitution and revocation and HEREBY RATIFYING AND
CONFIRMING all that my ATTORNEY-IN-FACT or his/her
substitute can lawfully do or cause to be done by virtue thereof.
