a. Disease-Modifying Anti-Rheumatic Drugs (DMARDs)

1. Etanercept

Available: Enbrel 25mg/ 0.5 ml solution for injection

Dose:

Adult w/ RA, psoriatic arthritis: 50mg once a week for 12 weeks

Ped plaque psoriasis (6-18 yrs old): 0.8 mg/kg once a week for 24 weeks

Juvenile idiopathic arthritis (2-18 yrs old ) 0.4 mg /kg twice daily

2. Infliximab

Available: Remicade 100 mg vial

Dose:

RA: 3mg/kg for 2-6 weeks

b. Hyperuricemia and Gout preparations

1. Allopurinol

Available: Allurase 100mg, 300 mg tablet

Dose:

Adult mild gout: 100mg-300mg/day

Moderate to severe gout: 300-600 mg/day

Severe hyperuricemia: Up to 300 mg per day

2. Febuxostat

Available: Furic 40 mg, 80 mg tablet

Dose:
Patients with gout : 40-80 mg once daily

c. Muscle relaxants

1. Baclofen

Available: Lioresal

Dose:

Adult: 15mg/kg/day

Children 0.3 mg/kg/day

Patient under hemodialysis: 5mg/ day

2. Tizanidine HCl

Available: Sirdalud 2mg tab

Dose:

Usual: 2-4 mg tid

Severe cases: 36 mg daily dose

e. Neuromuscular disorder

1. Pyridostigmine Br

Available: Mestinon 60 mg dragee

Dose:

Adult: 1-3mg bid-qid

Children: 7mg/kg/day at 4 hrly intervals

f. Other drugs acting on Musco-Skeletal System

1. Glucosamine sulphate

Available: Flexsa 1500 , 1500 mg powder for oral soln

Dose:

OA: 1 sachet daily

Patients (75 kg): 20 mg/ kg

2. Sodium hyaluronate

Available: Viscus 25 mg/2.5 ml soln for injection

Dose:

Adult 25 mg once a week for 5 weeks

VI. Hormones

a. Androgens and related synthetic drugs

1. Testosterone undecanoate

Available: Nebido 250 mg/ml injection

Dose:

1 amp every 10-14 weeks

b. Oestrogens, Progesterones and related synthetic drugs

1. Estradiol

Available: Estrofem 2mg tab

Dose: 1 tab daily w/o interruption

2. Dydrogesterone

Available: Duphaston 10 mg tab

Dose:

Dysmenorrhea: 10mg or 20 mg daily from day of the menstrual cycle

Endometrosis: 10-30 mg daily 5-25 day of the cycle

Dysfunctional uterine bleeding: 20 or 30 mg for 10 days

Secondary amenorrhea: 10 or 20 mg for 14 days

c. Combined sex hormones
d. Corticosteroid Hormones
1. Biocort

Available: Hydrocortisone Na succinate 500 mcg tab

Dose:

Adult: 100 -500 mg 3-4x a day

Children 6-8 yrs old: 100mg

1-5 yrs old: 50 mg

2. Prednisone

Available: Prolix 20 mg tab

Dose:

Immunosuppressive : 10-100 mg daily

Primary and secondary adrenocortical insufficiency: 5-25 mg

Adrenogenital syndrome: 10 – 20 mg

e. Trophic Hormones and Related synthetic drugs

1. Octreotide acetate

Available: Sandostatin 0.1 mg/ml , 0.5 mg/ml inj

Dose:

Acromegaly and gastro-entero-pancreatic endocrine tumors: initially 0.05 mg -0.1mg SC once or

bid

AIDS-related refractory diarrhea: 0.1mg SC tid

Complications following pancreatic surgery : 0.1 mg SC tid

2. Clomifene citrate

Available: Clomid 50 mg tab

 Dose:

Patients who had recent uterine bleeding: 50 mg daily for 5 days

VIII. Contraceptive agents

a. Oral contraceptives
1. Lynestrenol

Available: Daphne 500mvg tab

Dose: 1 tab daily beginning on the first day of menses.

2. Cypoterone acetate , ethinylestradiol

Available: Diane-35 500 mcg tab

Dose:

1 tab daily for 21 days

b. Depot contraceptives
1. Medroxyprogesterone acetate

Available: Depofemme 150 mg/ml susp for inj

Dose:

150 mg deep IM 3 mnthly

c. Other contraceptives
1. Copper T model (TCu 380A)

Available: Pregna IUD insetion kit

Dose:

Women having menstrual cycles: anytime w/in the 1st 12 days after menstruation

After childbirth: 10 mins after the delivery

After miscarriage or abortion: immediately if no infection present

VIII. Anti- Infectives

a. Aminoglycosides
1. Amikacin sulphate

Available: Amikacide 50 mg/ml , 125 mg/ml , 250 mg/ml inj

Dose

Adult, children, and older infant: 7.5 mg/kg 12 hrly

2. Gentamicin sulphate

Available: Mycin 40 mg/ml inj (amp)

Dose:
Adult: 1-1.7 mg/kg 8 hrly

Children: 2-2.5 mg/kg

b. Cephalosporins
1. Cefuroxime

Available: Cimex 500 mg FC tab, 125 mg/5ml , 750 mg powder for injection

Dose:

Adult Pharyngitis, Acute sinusitis: 250-500mg bid

Acute bronchitis: 250-500 mg bid

Uncomplicated gonorrhoea: 1g bid

Uncomplicated UTI: 125 mg or 250-500mg bid

2. Cefaclor monohydrate

Available: Ephron 500mg caps, 125 mg/ml, 250 mg/5ml granules for oral inj, 50 mg/ml oral drops

Dose:

Children (5 yrs old) : 10 ml

(1-5yr): 5 ml

(1yr): 2.5 ml

(1 month): 20 mg/kg
 c. Penicillins
1. Amoxicillin

Available: Amoxil 250 mg tab, powder for oral susp 125 mg/ 5ml

Dose:

Adult: 250 mg tid

Severe or recurrent purulent infection of the respiratory tract: 3g bid

Simple acute UTI: two 3g doses w/ 10-12 hr interval

Dental abscesses: two 3g doses w/ 8 hr interval

2. Ampicillin

Available: Ampicin 500 mg cap, 250 mg powder for inj

Dose:

Adult and children more than 20 kg: 250 -500 mg 6 hrly

Adult and children less than 20 kg:100 -200 mg 6 hrly

Infant more than 7 days: 75 mg/ kg/day 8 hrly

Infant less than 7 days: 50 mg/kg/day 12 hrly

d. Other beta lactam

1. Meropenem trihydrate

Available : Mepenem 500 mg powder for inj

Dose:

Adult: 0.5-1 g 8 hrly

Cystic fibrosis: 2g 8 hrly

2. Imipinem, Cilastatin Na
 Available: Vexpinem 20 ml powder for inj(vial)

Dose:

Adult 1.5-2g daily

Moderate infection: 500 mg 8 hrly

Severe infection: 6 mg 8 hourly

e. Chloramphenicols
1. Chloramphenicol Na succinate

Available: Klorfen 1g powder for inj (vial)

Dose: Adult 50 mg/kg/day 6 hrly

Children: 50 mg/kg/day in 4 divided doses

2. Chloramphenicol palmitate

Available: Penachlor 125 mg/5 ml oral susp

Dose:

Adult: 10 ml

Children 7-12 yr: 5ml

Infant: 1.25-2.5 ml

f. Macrolides
1. Azithromycin dihydrate

Available: Zithromax 250 mg, 500 mg FC tab, 200mg/5ml powder for oral suspension

Dose:

Adult: 500 mg single daily dose for 3 days

Children: 10mg/kg/day for 3 days

Community acquired pneumonia: 500 mg

 Pelvic inflammatory disease: 500 mg as a single dose by IV infusion for 1-2 days

2. Clarithromycin

Available: Macrodin 5oo mg FC tab

Dose:

Adult Chronic bronchitis: 500 mg 12 hrly 7-14 days

Pharyngitis or tonsillitis: 250 mg 12 hrly for 7-14 days

g. Quinolones:
1. Ciprofloxacin Na:

Available: Ciprofen 500 mg tab

Dose:

Adult: 250-750 mg bid

Meningococcal prophylaxis: 500 mg once daily

2. Levofloxacin
