Teaching Care Plan on Wound Care

A. DESCRIPTION OF THE LEARNER The learner is a 67 year old Female who is diagnosed with Splenic flexure colonic adenocarcinoma who has been in the hospital
since January 20, 2017. She had undergone exploratory laparotomy 8 days ago, and occasionally complains of a fully soaked dressing in the operation site which prompted
her to verbalize a need for learning proper wound management. She is a high school graduate from Nueva Vizcaya and is only able to understand Ilocano as a medium of
instruction.

C. LEARNING DIAGNOSIS Knowledge deficit: Wound care related to lack of exposure
D. GOAL After 15 minutes of health teaching, the client will be able to enhance their knowledge on how to aseptically care for her wound.

and discard the soiled one in
a plastic bag.
 Dry surrounding skin by
patting with a new gauze.
