DIRECT OBSERVATION PROCEDURAL SKILLS (DOPS)

Skill: Bag Valve Mask (BVM) Ventilation

Objectives:
1. To choose the appropriate mask size.
2. To demonstrate the correct technique of ensuring that all the valves are working well.
3. To demonstrate the correct technique for bag-mask ventilation.

2. Patient valve
a) Lip valve
- Place the reservoir bag over the patient valve.
Compress the self- inflating bag several times. This
will fill the reservoir bag and confirms that the
CSSC, Seremban
patient valve is able to efficiently direct air to the
patient.
b) Expiratory port = Disc membrane
- Compress the filled reservoir bag. Lifting of the
disc membrane confirms that air from the
ventilation bag has been vent to the atmosphere.
3. Reservoir flap valve
a) Overpressure relief valve
- Fill the reservoir bag with air again (follow step
2a). Quickly attached the filled reservoir bag to the
reservoir flap valve. Now compress the reservoir
bag. Lifting of the overpressure valve confirms the
venting of air from the reservoir bag to the
atmosphere.

b) Air entrainment valve


- Connect the reservoir bag to the reservoir flap
valve. Compress the self- inflating bag. Rapid re-
expansion of the self-inflating bag confirms intake
of air through the air entrainment valve.
3. Technique Assemble the bag-valve unit.
Choose the appropriate mask size
- Mask should fit from the nasal bridge till just
above the chin and covers the lips completely.

- Mask should not pressed on the manikin’s eyes.


Provide effective ventilation by ensuring
- misting in the mask
- visible chest rise
- no air leak around the mask
Without CPR
- Deliver 1 ventilation every 5 to 6 seconds (one
breath every 10 to 12 seconds)

With CPR
- Deliver 2 ventilation after every 30 compression
at a ratio of 30 : 2
4. Post- Remove gloves and face mask, dispose in
procedure appropriate bin and wash hands.
