been sworn in accordance with law hereby depose and state, that –

1. I was born and raised in Santa Cruz, Laguna.

2. I have no criminal record. I have no pending criminal or civil case

filed against me.

3. I execute this affidavit to attest to the fact that I have no derogatory

record whatsoever; to serve as supporting document for my
application for active duty at the Philippine Navy; and for any other
lawful purpose/s this may serve.
