Developing a Nursing Care Plan

Read the following scenario. By using the skills and resources available to you as a student
practical nurse, you will analyze assessment data using the metaparadigm concepts, establish
nursing diagnoses, write a goal/outcome statement using SMART criteria, and choose
interventions to achieve positive health outcomes. You will also provide rationale and literature
support for your choice of interventions. You will then plan how you will evaluate if the client
met the goal/outcome.

Scenario

Mr. Smith, 69 years old, is eight days post-operative for a colostomy. He complains of increased
pain at the surgical site. The previous nurse noted at the end of shift that the ostomy site was
leaking and the patient complained of burning around the skin. Mr. Smith has a past medical
history of IDDM (Insulin-Dependent Diabetes Mellitus) and requires blood glucose monitoring
to be done before meals.

Step 2: Create Nursing Diagnoses


Using the Nursing Diagnosis portion of the Nursing Care Plan Form, write two (2)
nursing diagnoses – one actual and one potential – that include the following:
 NANDA-approved nursing diagnosis statement. Make reference to a client strength
or health need. (Click here to access the List of NANDA Statements.)
 Cause/Contributing factors/Etiology (related to)
 Signs and Symptoms/Presenting evidence (as evidenced by, if applicable).

Note that the nursing diagnosis statement is one sentence that contains all three
components.

Step 3. Create Nursing Care Plan/Planning Goals and Interventions


Use the Nursing Care Plan Form to complete the care plan based on the scenario.
Priority Diagnosis:
 Choose one of the diagnoses you have written as your priority. Include why you
have made it a priority.
Goals/Expected Outcomes:
 Write one goal/outcome statement using SMART criteria. Be sure to include all of
the key components. Note that the goal/outcome statement should contain all of
the SMART criteria in one sentence.
 Document the goal/outcome statement on the Nursing Care Plan form.

Evaluation:
 Identify how you plan to determine if the stated goal/outcome was achieved or
not. The evaluation statement should match the goal/outcome statement and
provide support with evidence from the literature. Be sure to include your sources
on a reference page. The evaluation statement needs to be clear and detailed
enough that any practical nurse would be able to evaluate the outcome.
Interventions:
 Identify three nursing interventions to help the client achieve the goal/expected
outcome, with evidence from literature, that relate back to your nursing diagnosis
and that will lead to a successful outcome and resolution of your nursing diagnosis.
 You need to ensure your interventions are from a nurse's perspective: “The nurse
will...” “What would you, a nurse…?”
 Document your nursing interventions on the Nursing Care Plan form and provide
support for your interventions (why you are choosing the particular intervention
with evidence from the literature. Be sure to cite your references as per APA
guidelines and include your sources on a reference page.
