CASE STUDY 1
SITUATION:
Mr. Lee Min Ho a 65-year-old man who was brought to RPGMC from a long-term care
facility by the rescue team after complaining of severe dyspnea and shortness of breath.
He has been experiencing coldlike symptoms for the past 2 days. He has a productive
cough with thick greenish sputum. When Mr. Min Ho awoke in the nursing home he found
that he was having difficulty of breathing even after using his albuterol (Proventil) metered
dose inhaler (MDI). he appears very anxious and is in respiratory distress. His history
includes chronic obstructive pulmonary disease (COPD) related to smoking of 2 packs of
cigarettes per day since he was 15 years old. He has been incontinent of urine and stool
for the past 2 years.
In the emergency room, the patient undergoes chest radiography and admission
laboratory are performed including electrolyte levels and complete blood count. A sputum
sample is sent to the laboratory for culture and sensitivity testing and gram staining. His
vital signs are as follows:
BP: 153/92 mmHg
HR: 118 bpm
O2 sat: 88% on 1 Lpm oxygen via nasal cannula
RR: 38 bpm
Temp: 38.3 C
