AFFIDAVIT

I, JOASH ARVHY JOY J. BARCINAS, of legal age, Filipino, single and

with residence address at #1129 3RD Street, Carebi Subdivision, Barangay Sto.
Niño, Angono, Rizal, after having been duly sworn to in accordance with law,
hereby depose and say, that;

1. I finished my Secondary Schooling at Angono National High

School, Angono, Rizal in year 2014;

2. I stopped schooling during year the immediately proceeding school

year up to April 2018;

3. I was never enrolled in any college, university and/or vocational

training institution, otherwise stated, I have not been admitted to
college since my high school graduation;

4. I am executing this affidavit to attest to the truth of all the

foregoing, for all legal intents and to satisfy the documentary
requirements of the University of the Philippines (UP), Diliman,
Quezon City and for whatever legal purpose/s it may serve.
