Introducción
Los antibióticos betalactámicos son la causa más frecuentemente reportada de alergia a fármacos en todo
el mundo. Este tipo de alergia representa una causa importante de muerte por anafilaxia, representando a
más de un 20% de las muertes en Europa y más de un 75% en Estados Unidos.
Las posibles razones por las cuales los pacientes supuestamente alérgicos toleran las penicilinas son: que
el paciente haya sido mal diagnosticado, los síntomas fueron provocados por la enfermedad subyacente o
la interacción de la enfermedad y el fármaco (cuadros virales como principal causa de urticaria en niños,
o interacción del fármaco con el antibiótico, ej.: amoxicilina + virus de Epstein Barr) , los niveles de inmu-
noglobulina E específica van disminuyendo con el tiempo (no siempre sucede) y el paciente se curó de su
alergia.3
Los costos médicos de aquellos pacientes con un diagnóstico erróneo son mayores4, hasta un 63 % más
en comparación con los no alérgicos5. Esto se debe a que habitualmente estos pacientes deben utilizar
antibióticos más caros y de más amplio espectro. Además el uso de antibióticos de amplio espectro es un
factor de riesgo de resistencia bacteriana 6. También deben tenerse en cuenta el impacto en la calidad
de vida de los pacientes y las familias o cuidadores, los miedos innecesarios y el uso de antibióticos gene-
ralmente de menor tolerancia por el niño, debido a otros efectos adversos o simplemente el sabor de las
formulaciones
