HSE PROJECT CLOSEOUT REPORT TEMPLATE —- ENGINEERING / FIELD

PURPOSE

This procedure defines the process to be used at the completion of a project to ensure
relevant information is captured, documented, and reported – including lessons learned
for consideration by other projects.

SCOPE

This procedure includes a General Requirements section.

APPLICATION

This procedure applies to work activities and employees under the control of ------- and
its contractors.

HSE PROJECT CLOSEOUT REPORT TEMPLATE —- ENGINEERING / FIELD

1.0 GENERAL REQUIREMENTS

Using Form 000.100.F0681, Project Completion Report — Template, project/site


management and HSE will collaborate in identifying pertinent project HSE-related
information, achievements, and lessons learned.

Refer to Attachments 01 and 02 for sample HSE closeout report templates. These sample
reports are not intended to be a true representative of a complete report, but intended to
be used as a starting point. The development of closeout report templates, based on these
templates and any exceptions, should be approved by the Project Manager, Business Unit
HSE Lead, and client for the project.
