JOINT AFFIDAVIT
We, ELISA Q. VICADA, married, and ROSITA L. ZAMORA, married, both of
legal age, Filipinos, and residents of Highway, Poblacion, Alegria, Cebu, after
having been duly sworn to according to law, do hereby depose and say that:

1. We personally know MARIA TERESA VILLARUBIA COMON-

SAD who is our close friend, neighbor and the daughter of the
deceased spouses Abundio Villarubia Sr. and Purita Gomez Vil-
larubia, residents of Municipality of Algeria;

2. We came to know just lately that both her SSS and PAG-
IBIG records input an erroneous information regarding her name,
her SSS and PAG-IBIG records misspelled the first name as THERE-
SA (reflected in her SSS records) instead of MARIA THERESA (as re-
flected in her NSO Birth Certificate);

3. We are executing this affidavit to attest to the fact that

the names THERESA (as reflected in her SSS and PAG-IBIG records),
and MARIA THERESA (as indicated in her NSO Birth Certificate) all
refer to ONE and the SAME person—;

4. In executing this affidavit, we are fully aware and con-

scious of our duty and obligation to state only such facts as are
true and correct of our own knowledge, as well as of the legal
sanction for swearing to any falsehood under oath; and

5. We hereby affirm under pain of perjury that our forego-

ing statements are all true and correct.
