L’allongement coronaire, également appelé élongation coronaire, est un acte destiné
à augmenter la couronne clinique d’une dent.

Introduction
Les allongements coronaires ou élongations coronaires sont des procédures chirurgicales
particulièrement fiables si elles sont bien maîtrisées. En effet, de nombreux paramètres
sont à respecter afin d’obtenir une restauration conservatrice ou prothétique optimale. Ces
actes d’allongements coronaires sont en général réalisés pour restaurer la physiologie du
sulcus suite à un délabrement coronaire sous-gingival ou suite à des impératifs esthétiques
(alignement des collets par exemple). Ils constituent souvent une alternative aux extractions
ou à des traitements plus lourds et coûteux, comme par exemple, une restauration
prothétique implanto-portée.
Les inserts ultrasonores d’allongement coronaire ont été spécialement conçus dans le but

3. Espace biologique
Selon GARGIULO et coll. (1961), qui l’a défini, l’espace biologique est compris entre
le fond du sulcus et le sommet de la crête osseuse. Il mesure en moyenne 2,04 mm.
La hauteur du sulcus est quant à elle estimée à 0,69 mm. La somme des deux, arrondie
à 3 mm, constitue la hauteur moyenne de la zone comprise entre le sommet de la crête
marginale et la crête osseuse, soit l’espace chirurgical pré-prothétique à préserver.

Applications cliniques
La modulation des fréquences permet de limiter la lésion du ligament ainsi que des
fibres desmodontales qui, selon Glise J-M. et Monnet-Corti V. (11), accélèrent la
cicatrisation et réduisent les complications postopératoires. Les fibres desmodontales
sont donc visibles et donnent un aspect de duvet.
Cette sélectivité de coupe offre à l’insert ultrasonore une innocuité totale sur la
gencive. Ceci peut être particulièrement bénéfique à la suite d’une gingivectomie à
biseau interne par exemple, où le risque
d’endommager les tissus mous est relativement
important avec une instrumentation rotative.
De plus, le risque d’atteinte du tissu radiculaire
est moindre. Moins invasifs, les inserts
ultrasonores offrent beaucoup plus de confort,
limitant le risque d’abîmer l’émail de la dent
traitée et celui des dents voisines.

Le phénomène de cavitation (apparition de micro-bulles), permet de bénéficier d’une
visibilité optimale du champ opératoire. Régulé, ce spray stérile évite toute élévation
thermique susceptible d’entraîner une dégradation tissulaire.
Enfin, travailler dans une atmosphère sereine n’est pas négligeable. Le bruit engendré
pour une telle chirurgie sera semblable à un détartrage aux ultrasons et sera donc plus
confortable, pour le patient et le praticien, que l’utilisation des instruments rotatifs.

Cet insert est notamment dédié aux ostéoectomies de l’os de soutien en zones
interproximales et péri-radiculaires vestibulaire et palatine. La partie
travaillante de 3 mm de hauteur permet de supprimer l’os excédentaire et de
mesurer pendant l’ostéoectomie les 3 mm nécessaires au respect de l’espace
biologique. L’insert CE3 peut être utilisé perpendiculairement ou parallèlement

Crown lengthening, also called crown elongation or crown extension, is a surgical
procedure performed to expose a greater height of the clinical crown of a tooth.

Introduction
Crown lengthenings are particularly reliable surgical procedures, when performed correctly.
Indeed, numerous parameters must be respected to obtain an optimal conservative or
prosthetic restoration. These crown lengthening procedures are generally performed to restore
the physiology of the gingival sulcus further to subgingival crown destruction or esthetic
requirements (gingival contouring for example). They often present an alternative to
extractions or more complicated and more expensive treatments such as implant-supported
prosthetic restorations for example.
The ultrasonic tips for crown lengthening have been specially designed with the aim to address
the difficulties encountered by traditional instruments and to allow more delicate and
more precise manipulation. This booklet presents the advantages of this new technique and
the clinical protocols explaining how to use it.

Principles
3. Biologic width
The biologic width as defined by GARGIULO et al. (1961) is measured from the bottom
of the gingival sulcus to the alveolar crest. This dimension is approximately 2.04 mm.
The mean depth of the gingival sulcus is 0.69 mm. The sum of these two figures, rounded
off to 3 mm, constitutes the space occupied between the summit of the gingival margin
and the alveolar crest, the pre-prosthetic surgical space to be preserved.
Respect of the biologic width is essential during treatment and restorative or prosthetic
procedures. Not respecting it can engender periodontal lesions such as gingivitis,
gingival recession and bone resorption. In order to increase the clinical crown height
and restore a healthy physiology, the alveolar crest must be apically positioned at
3 mm with regard to the apical finish line of the preparation. The biologic width can
therefore physiologically be organized while the access to the cervical finish line of
the preparation is improved. The alveolar crest level will determine the final position
of the marginal gingiva.
