CHAPTER 1 GENERAL ASSESSMENT


Part 1 (Collecting Demographic Data Elements)
Nurse : “Good Morning Miss..”
Patient : “Good Morning Sir”
Nurse : “I’m Nurse Yoga on duty today..I just want to interview you for assesment.
Are you ready miss?”
Patient : “Yes, I’m ready sir”
Nurse : “Okay, what is your complete name?”
Patient :”My name is Ulmaifa Nanda Putri sir, you can call me Nanda”
Nurse :”Okay Miss Nanda, where do you live ?”
Patient : “I live in Ngunut Village, Tulungagung.”
Nurse :”How old are you miss ?”
Patient :”I’m 20 years old sir”
Nurse : “What is your phone number?”
Patient : “My phone number 082334407709”
Nurse : “Are you married?”
Patient : “ No, I am not yet married.”
Nurse : “What is your religion?”
Patient : “My religion is islam.”
Nurse : “Do you have any health insurance?”
Patient : “No, I don’t have any health insurance.”
Nurse : “Do you have any academic title?”
Patient : “No. I don’t have academic title.”
Nurse : “What do you do?”
Patient : “I am a student in Health Science Institute Bhakti Wiyata Kediri”
Nurse : “Who is your next of kin?”
Patient : “My next of kin is my mother. Her name is Mrs. Lastri”
Nurse : “What makes you come to this hospital?”
Patient : “ I want to check my stomach sir, I feel so pain in my stomach and i think
there is a problem. It makes me worried so i come to this hospital”

Part 2 (Current‐Health and Illness Status)


Nurse : “What do you think about your health?”
Patient : “ I think my health is passably before, but for now my stomach ache so
disturbing me and make me uncomfortable”
Nurse : “Would you tell me about your fecal elimination?”
Patient : “I think my fecal elimination in good condition. The consistency is solid,
and the smell of the typical chapter.”
Nurse : “How many times a day do you do your fecal elimination?”
Patient : “I do fecal elimination about 1-2 in day”
Nurse :”Is stool formed or lossed?”
Patient :”It’s formed sir”
Nurse : “Do you have any problem with your urination?”
Patient : “No, urination is normal .”
Nurse : “Is your urination sluggish?
Patient : “No, my urination is normal.”

Part 3 ( History of Past Health and Illnesses)


Nurse : “Have you ever had gastritis?”
Patient : “Yes, I have.”
Nurse : “How old were you when you got it?”
Patient : “Since I was 10 years old sir”
Nurse : “Are you allergic to a some food?”
Patient : “No, I’m not sir”
CHAPTER 2 DIMENSION OF SYMPTOMS
Nurse : “Where do you feel it?”
Patient : “I feel the pain around my stomach sir, it’s so uncomfortable”
Nurse : “Does it move around?”
Patient : “No, it does not.”
Nurse : “Show me where.”
Patient : “In here sir”
Nurse : “What is it like? Is it sharp, dull, stabbing, aching?”
Patient : “I think it’s like a shrap pain sir”
Nurse : “When did it last?”
Patient : “When I’m walking here”
Nurse : “On a scale of 0 to 10, with ten the worst, how would you rate what you feel
right now?”
Patient : “I think the pain on 6 scale.”
Nurse : “Does this interfere your usual activities? In what ways?”
Patient : “Yes it does sir, in ways I do hard activities”
Nurse : “When did you first notice it?”
Patient : “I feel the pain since 3 days ago.”
Nurse :” How long does it last?”
Patient :”Until I take a rest sir”

Nurse : “Does it occur in a particular place or under certain circumstances?”


Patient : “Yes it does sir, it occur just in this area and happen when i do hard
activities”
Nurse : “Have you taken anything for it?”
Patient : “No I have not sir. I scared to by medicine if not from hospital”
Nurse : “Thanks you for your cooperation miss. please to rest back and I hope you
recover quickly. The doctor will check you soon”
Patient : “You are welcome sir, I will wait doctor’s coming”
