CERTIFICATION GUIDELINES
Please check the spelling of your name on the list to be reflected on your UAPSA National
Membership card. If you find your name misspelled or contains any kind of typographical error,
please do not hesitate to inform any of your chapter officers through their contact numbers indicated
below:

NOTE: The character Ñ will be reflected as N on the actual membership card. The name that will
appear on the membership card will be limited to 26 characters only, including spaces, periods (.)
and dashes (-). Decide on the characters to be omitted for names that exceed the limit. If the
following conditions are not met, the machine will invalidate the excess characters resulting to an
incomplete name. The character Ñ will be left as □ if unchanged. The name on the card would be
printed in this format:

First Name, Middle Initial, Last Name

Furthermore, spaces left blank would be assumed to be certified true and correct. This means
that if the space provided for corrections is left blank, it is understood that no alterations are to be
made.

If no responses are submitted a week after this notice is released, then UAPSA National will
not be held liable or responsible for any errors/misinterpretations that will reflect on the membership
card.
