Program Terms and Conditions
Please read carefully all the following information before signing.
COST: The cost of this program is $1275 U.S. dollars. Early reservations are
strongly recommended, and a non-refundable deposit of $200 per person is required
at the time of booking, with the remaining balance due May 31. Applications received
after this date should include full payment. Final documents will not be confirmed
until final payment is received. The following chart is a minimum payment
recommendation:

**If payments are late, a late fee of $5 per day will be charged, unless prior
arrangements have been made with the directors.
PARTICIPANT CANCELATION POLICY: All cancellation penalties are the
responsibility of the participant. With the many recent changes in the travel industry
such as the issuing of non-refundable group airline tickets. To compensate for

PROGRAM CHANGES: Sacred Ground Tours reserves the right to make changes or
substitutions in the interest of the program in order to meet unexpected situations
with the itinerary.

TOUR COST: The cost of the tour is based on tariffs, charter bus rates,
accommodation rates, and rates of exchange quoted to Sacred Ground Tours at the
time of publication. These prices are expected to be in effect at the time of
departure; however, any changes in price made by the airlines, hotels, vendors, and
operators, will be passed on to you - the participant. If such rates change prior to
departure, Sacred Ground Tours is not under any obligation to pay extra expenses
arising from the changes. The resultant cost increase will be billed to each
participant’s account and will require immediate settlement.
PROGRAM CANCELLATION: Sacred Ground Tours reserves the right to cancel the
program, if deemed necessary for reasons including insufficient program participation
and circumstances beyond Sacred Ground Tours control, such as strikes, lockouts,
riots, wars, whether declared or undeclared, hostilities, civil disturbances, acts of
God, acts of governments or other authorities, thefts, pilferage, epidemics,
quarantines, custom regulations, delays, or cancellations of or changes in itinerary or
schedules or from any causes beyond Sacred Ground Tours control. In such event,
Sacred Ground Tours shall have no responsibility, beyond the refund of deposits and
monies paid to Sacred Ground Tours by the participants. A refund will then be made
to participants, within 30 days, by means of a company or bank check.
LOSS AND DAMAGE: Sacred GroundTours or any associated representative, acts
as agent for the hotels, airlines, ship companies, or owners or contractors providing
accommodations, transportation or other services. Contracts and tickets are issued
subject to any and all tariffs, terms and conditions under which any services
whatsoever are provided by such vendors or contractors. Sacred Ground Tours or
any associated representative shall not be or become liable or responsible for any
loss or injury or damages to person, baggage, property or for any additional
expenses or liability sustained by the traveler in connection with any services
resulting directly or indirectly from any acts described above. The tickets or contracts
in use by any vendor or contractor providing any services shall constitute the sole
contract between such vendor or contractor and the purchaser of tour and/or tour
member.
HEALTH AND INSURANCE: We advise each participant to have a physical
checkup before registering for the program. Sacred Ground Tours cannot be
responsible for illnesses that may occur while traveling. Since group sightseeing can
be physically taxing, we recommend that persons who suffer from any physical
impairment, which could hinder them from normal participation, contact Sacred
Ground Tours prior to submitting an application.
I hereby grant permission for emergency first aid to be administered if deemed
necessary. Sacred Ground Tours does not provide medical and accident insurance.
If an emergency or health crisis should occur on the tour, medical assistance will be
secured, however, the cost of such medical assistance will be the responsibility of the
participant. Baggage, cancellation, and personal insurance are strongly
recommended through private insurance companies.

DEATH: Sacred Ground Tours shall not be liable for any death, nor for any damages
or claims arising from negligence of any person not its employee or under its
exclusive control.
LDS STANDARDS: All participants are required to maintain ideals and standards in
harmony with those of The Church of Jesus Christ of Latter-day Saints. High
standards of honor, integrity, morality, and abstinence from narcotics, alcohol,
tobacco, coffee, tea, etc., are mandatory. As a participant in this program, I shall:
1. Support and uphold the moral standards of the Church of Jesus Christ of
Latter-day Saints.
2. Dress modestly at all times. I understand that clothing is inappropriate when
it is sleeveless, strapless, backless, or revealing and that dresses, skirts,
and shorts must be knee length or longer.
3. Accept the suggestions and direction of the program director in all matters
pertaining to the program or personal conduct, imposed curfews, health, or
hygiene.
4. Strive to be friendly toward all members of the group.
5. Accept the will of a majority of the group whenever a matter of choice
presents itself.
If I refuse to follow these guidelines, and any other instructions of the tour director, I
will fully indemnify Sacred Ground Tours for any loss suffered as a result of my
behavior.

PARENTAL SUPPORT STATEMENT:I understand that the group leaders will be
Ryan and KaDawn Webb and I will respect their judgment and encourage my child to
follow their counsel, respect them and their authority and stay out of trouble. I will
insist that my child adhere to the LDS Standards listed above. I understand that if my
child breaks this code of conduct during the trip, the above-named group leaders will
contact me and I will assist them in determining what to do what would be in the best
interest of the group. I understand that if my child breaks the code of conduct
(above), the group leaders have the right to send my child home at my expense.

TRANSPORTATION: I understand and agree that all tickets are issued subject to
any and all terms and conditions under which means of transportation or other
services are offered and provided by the issuer, and that Sacred Ground Tours, in
the absence of negligence, shall not be or become liable or responsible in any way in
connection with such transportation or other such services, or for any loss, injury or
damage to, or in respect of, any person or property howsoever caused or arising, or
for damages resulting from any other cause connected directly or indirectly with the
program for which application is hereby made.
C.E.S. AFFILIATION: Although the directors of this program are employees of the
L.D.S. Church Educational System, I hereby acknowledge my understanding that this
program is not affiliated with the Church Educational System in any means and shall
save and hold harmless the Church of Jesus Christ of Latter-day Saints, the LDS
Church Educational System, the directors of the program, and any other individual or
organization connected with this tour.
AGREEMENT AND SIGNATURE(S): Having read carefully this information
concerning the program, I hereby submit my application (2 pages with signatures) for
membership in the program and agree to comply with the guidelines and regulations
of this program. I understand that said application must be approved by Sacred
Ground Tours and the directors. With this application, I include my non-refundable
$200 deposit (payable to: Sacred Ground Tours LLC.) and agree that full and final
payments will be submitted according to the payment schedule.
