DEED OF ASSIGNMENT

KNOW ALL MEN BY THESE PRESENTS:

This DEED OF ASSIGNMENT, made and entered into by and between

______________Filipino, of legal age, single/married, resident of and with postal
address at _________________, , hereinafter called the ASSIGNOR; and .,
______________________________, a private financial institution duly organized and
existing under and by virtue of the laws of Philippines, with principal place of
business at Poblacion, ____________________, represented by
________________________, hereinafter called the ASSIGNEE;
WIINESSETH:

WHEREAS, the ASSIGNOR has been granted certain loan/s, cash advance/s
and other credit accommodation/s by the ASSIGNEF and shall be granted
additional and other loan/s, cash advance/s and other credit accommodation/s
by the ASSIGNEE in the future;

WHEREAS, the ASSIGNOR is willing assign in favor of the ASSIGNEE by way of

pledge or collateral security to secure the prompt and full payment said loan/s,
cash advance/s and other Credit accommodation/s granted by the ASSIGNEE
to the ASSIGNOR, as well as those that the ASSIGNEE may hereafter grant or
extend to the ASSIGNOR, the following:

subject to the following terms and conditions:

NOW, THEREFORE, in consideration of the premises and mutual covenants

herein contained, the ASSIGNOR, by way of pledge, delivers to deposit/s with the
said ASSIGNEE the afore-mentioned personal property of his/her own exclusive
ownership as a collateral security for the payments loan/s, cash oi advance/s and
other credit accommodation/s granted by the ASSIGNEE to the ASSIGNOR, as
well as those that the ASSIGNEE may hereafter grant or extend to the ASSIGNOR,
wherein the ASSIGNOR hereby authorizes the ASSIGNEE, or its authorized
representative, to withdraw from Savings Account/Current Account by way of
the ATM Card described above and apply such withdrawal/s as payment, either
partial or full .as the case may be, to the said existing obligation/s of the ASSIGNOR
with the ASSIGNEE as well as those that the ASSIGNOR may hereafter obtain from
the ASSIGNEE.

1. That the said ASSIGNOR shall remain liable for any outstanding balance
of the aforementioned obligations, existing and those he/she may hereafter be
granted, if the ASSIGNEE is unable to actually receive the assigned salaries/sum/s
for any other cause whatsoever;
2. That ln case ASSIGNEE fails to received his/her salary by reason of
dismissal, administrative charge, suspension or any other cause resulting in the
withholding or forfeiture of salary/benefits, then the ASSTGNEE reserves the right
to collect from Assignor, real property/ies, all sum/s or money or cash deposits in
any other Bank/office to pay the unpaid balance, interests plus the expenses
incurred In the enforcement of such obligation/s;

3. THAT SAID ATM CARD SHALT NEITHER BE SOUGHT TO BE REPLACED NOR

AFFIDAVIT OF LOSS OF THIE SAID ATM CARD SHALL BE EXECUTED BY THE ASSIGNOR
TO THE DRAWEE/DEPOSITORY BANK WITHOUT THE EXPRESS CONSENT OF THE
ASSIGNEE. THAT FAILURE ON THE PART OF THE ASSIGNOR TO SECURE EXPRESS
CONSENT FROM THE ASSIGNEE IN REPLACING SAID ATM CARD OR IN EXECUTING
SAID AFFIDAVIT OF IOSS OF ATM CARD, WHILE THIS CONTRACT IS STILL
ENFORCEABLE, SHALL RENDER THE ASSIGNOR CRIMINALLY LIABLE OF PERJURY AND
SUCH OTHER CRIMES THAT MAY BE ASCRIBED TO IT;

4. That this Assignment shall be irrevocable until the aforementioned

obligation/s is/are fully paid.
