AFFIDAVIT OF WITNESS

I, CRISELDA BUHAY P. MENDOZA, of legal age, Filipino, single and a

resident of No. 21 Purok Maganda, Zone 7 Barangay Macarena Pasay City, after
having been sworn to in accordance with law, hereby depose and say that:

1. I am the General Manager of Vibers Club located at No. 17 Kaligayahan

Street, Leveriza Pasay City since January 10, 2015.

Attached are Certified True Copies of my Employee Profile and Company

ID marked as ANNEXES “1” and “2”, respectively;

2. I hired Miles Libres as a Guest Relations Officer last July 21, 2018 after
she submitted her pre-employment requirements.

Attached are Certified True Copies of her Bio-Data, Birth Certificate and
Employee Profile marked as ANNEXES “3”, “4”, and “5”, respectively;

3. On August 9, 2018, Miles reported to work at around 6 o’clock in the

evening and went home at 1 o’clock in the morning of August 10, 2018.

Attached is a Certified True Copy of Vibers Club’s Employee Logbook

marked as ANNEX “6”;

4. I am executing this affidavit to attest the truth of the foregoing facts and for
whatever purposes this may serve.
