AFFIDAVIT.
We, ARTHUR MARTIN ANG’IDO OGOLA of ID NO:20818210,and BENSON ORIDI
OGOLLA of ID NO:13528321 and BAUSTINE KHAYIKWA CHEMASA of ID
NO:10614053, of post office Box 6173-00200, Nairobi within republic of Kenya do
hereby make Oaths and state as follows:-

1. THAT we are Kenyan adults of sound minds and therefore competent to swear
this affidavit.

2. THAT we are duly registered proprietors of MAELI HOLDINGS LIMITED PIN

No. P051431349U which firm was registered under the Registration of
Company Act.

3. THAT we are applying for Customs Agent’s Licence for the year 2016 thus
requesting the Commission of Customs & Board Control to issue us with the said
Licence.

4. THAT we shall adhere to all the laid down procedure and we shall be
responsible for any liabilities of the Company.

5. THAT we swear this Affidavit in support of our application for Customs Agent’s
Licence for the year 2016.

6. THAT we swear this affidavit conscientiously believing everything contained

herein is true and in accordance with Oaths and statutory declaration Act CAP 15
of the laws of Kenya.

7. THAT we have deboned to herein is above is true to the best of our knowledge
