AFFIDAVIT OF ACKNOWLEDGMENT

OF FINANCIAL OBLIGATION
WITH PROMISE TO PAY

I, ELVIRA P. ESPINO, of legal age, married, Filipino citizen, and a

resident of Barangay Ungot, Tarlac City, after having been duly sworn to in
accordance with law, do hereby depose and say:

1. I am indebted to MS. MARLENE ESTEBAN of Barangay Ungot,

Tarlac City, in the amount of SIXTY THOUSAND FOUR HUNDRED
(P62,400) which I incurred on 17 January 2016;

2. The above-named creditor parted with her hard-earned money based

on my assurance that the said amount of indebtedness would be paid
not later than September 17, 2016 and should I fail to do so, I would
voluntarily turned over to her my employer issued ATM card against
which she (the creditor) my employer issued ATM card;

3. I admit that I have not yet settled such indebtedness but despite of
that, I have not turned over/surrendered to her (creditor) my employer
issued ATM card;

4. In view of the aforesaid acts, I acknowledged to have defaulted and

committed decent against creditor and in order to convince her
(creditor) not to file any legal action against me arising therefrom, I
have agreed to settle such overdue financial obligation on installment
by paying the amount of ONE THOUSAND PESOS (P1,000) every
15th and 30th day of the month starting 30 March 2018 until the total
amount of my subject indebtedness shall have been fully paid but
said creditor reserves the right to initiate appropriate legal action
against me in case I incur default in the payment of any installment.
