In anticipation of any eventuality during the observance of the celebration of the All Saints’ Day and
the All Souls’ Day in November 1 -2, 2010, CODE WHITE ALERT is raised in this office effective
October 30 – November 2, 2010. The following shall observe the schedules as follows:

Roles & Responsibilities:
1. All Designated Officers and Drivers of the Day shall render duty as stated in the
DOH-EV S.O. No. 53 dated Sept 15, 2011.
2. CHD-EV HEMS OPCEN & Response Team Leaders and Members, HEMS
Coordinator and Members of the Regional Mgt Crisis Committee shall be
ON-CALL on a 24/7 basis. (Refer to Directory ). However, upon assessment of
the situation shall render duty at a designated OPCEN upon the orders of the
Regional Director.
3. PHTO shall activate HEMS Team based on Regional S. O. No. 38-A dated July 29,
2010. They will render duty ON-CALL on a 24/7 basis. However, upon
assessment of the situation shall render duty at a designated OPCEN upon the
orders of the Regional Director.
6. All personnel on duty shall REPORT health emergency and disaster incidents
guided by the HEMS Reporting Protocol (p. 61) using Standard HEMS Reporting
Forms (p. 85-123).

7. All personnel who rendered duty during the holidays or beyond office hours shall be
entitled to COMPENSATORY DAY-OFF subject to joint CDC-DBM Cir No. 2, 2004.
