AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, CORAZON A. ALVAR and CRISTINA V. GUTIERREZ, both

of legal age, Filipinos, and residents of Bauan, Batangas and Calicanto,
Batangas City respectively, after having been duly sworn to in accordance
with law hereby depose and state that:

We personally know one Ma. LHEAN MANLAPAS LOTERTE born

on June 22, 2005 to Irene Drio Manlapas and Joemar Pasaugue Loterte
since she is the grandchild of PURISIMA DRIO MANLAPAS, a long time
client of the Public Attorney’s Office who frequently visits our Office;

We know that the mother’s surname of LHEAN is MANLAPAS and so

the middle name of LHEAN MANLAPAS LOTERTE should have been
MANLAPAS and not MALAPAS as what has been entered in her
Certificate of Live Birth;

We attest to the fact that the person whose name appears as Ma.
LHEAN MALAPAS LOTERTE in her Certificate of Live Birth is the same
person of Ma. LHEAN MANLAPAS LOTERTE;

That we are not related to Ma. LHEAN MANLAPAS LOTERTE by

consanguinity or affinity;

That we are executing this affidavit to prove the foregoing matters.
