A. ADOPTIVE PARENTS

1. Marriage Certificate
2. Birth Certificate of children
3. Latest Income Tax Return/Any Proof of Income
4. Affidavit of Consent (to Adoption by children age10 years and above)
5. Police Clearance/Barangay Clearance/Good Moral Character
6. Medical Certificate of couple and family members (Physically fit)
7. Psychological Evaluation
8. 3 letters of character reference
9. Family photograph
10. Picture of minor with adoptive parents
11. Home study report from licensed adoption agency in their place of residence of foreigner.
12. Affidavit of Consent (to adoption from biological parents)
13. Affidavit of Support (from adoptive parents)
14. Properties of adoptive parents (Bank deposit, Land ownership, Ownership of Motor vehicle)

B. ADOPTIVE CHILD/REN:

1. Birth Certificate of Founding Certificate

2. Marriage Contract of parents
3. Medical Certificate (Physically fit)
4. Affidavit of Consent of Birth parents
5. Affidavit of consent of minor adoptee age 10 years and above
6. Recent picture of minor
7. Child Study Report
