request to BIR for an extension of time to submit the additional requirement for the
issuance of a Certificate of Tax Exemption. Kindly therefore sign the request letter
and submit to us the soonest possible time together with the following additional SEC
requirements:

1. Financial Statements for 2011-2012 and GIS 2012 duly stamped “received” by the SEC
2. Photocopy of page of Membership Book (color red) stamped by the SEC;
3. Photocopy of DepEd Recognition
(As per attached list furnished by the SEC Monitoring Department)
