These procedures in the organization exposition are approved by the undersigned and should be complied with,
as applicable, when work orders are being progressed under the terms of the CAR 145 approval.
It is accepted that these procedures do not override the necessity of complying with any new or amended
regulation published by the GCAA from time to time where these new or amended regulations are in conflict
with these procedures.
It is understood that the GCAA will approve this organisation whilst the GCAA is satisfied that the procedures are
being followed and work standards maintained. It is further understood that the GCAA reserves the right to
suspend, limit or revoke the approval of the organisation if the GCAA has evidence that procedures are not
followed or standards not upheld.

INSTRUCTIONS:
i. A maintenance organization applying for GCAA AMO Approval with the ratings approved under
EASA/ FAA (with EASA Bilateral Agreement) scope of works. The Accountable Manager shall make a
declaration to the GCAA (required to complete Section A: only), OR
ii. A maintenance organization applying for GCAA AMO Approval with the ratings outside the EASA/
FAA (with EASA Bilateral Agreement) scope of works. The Accountable Manager is required to
make a declaration to the GCAA, (required to complete Section A and Section B)
b. Instructions: The following instruction must be followed to complete the Table in Section B of this form (when
applicable).
1) Airport Name (IATA Code)/ facility location:
Enter clearly the location of the maintenance facility where the intended maintenance works is being
performed.
e.g. - i: If located at Airport - Abu Dhabi International Airport (AUH), Norway House Airport
(YNE)
e.g. - ii: if located away from the Airport - Nantgarw, Cardiff, South wales, UK.
2) Class Rating 1:
Enter clearly define the class rating from one or more of the following; A, B, C and D.
3) Scope of Work1:
Enter clearly the level of the maintenance work performed as per the application submitted to the
GCAA (Refer Keys):
i. For Line maintenance - Level of Scope of Works: L1, L2. L3, L4 or L5
e.g. : L3- means L2 checks up to and including Weekly/Service check
: L5- as defined in the application submitted to the GCAA, and
: Any restriction/limitation applied, for e.g., Riding Engineer agreement (if any).

5) Remarks:
i. (*) Scope of Work outside EASA: Maintenance Organization applying for approval of station/ rating
not covered under EASA scope of work.
ii. (**) Scope of Work outside FAA (with EASA Bilateral Agreement): Maintenance Organization
applying for approval of station/ rating not covered under FAA (with EASA Bilateral Agreement)
