AFFIDAVIT OF LOSS

I, RAMEL S. OLAYVAR, Filipino, of legal age, married and a

resident of San Roque, Macrohon, Southern Leyte Philippines, after
having been duly sworn to in accordance with law, do hereby
depose and say:

1. That I have completed my Secondary Education at San Roque

National High School located at San Roque, Macrohon,
Southern Leyte;

2. That relative thereto, a High School Diploma was issued in my

favour by the said school;

3. That, I remember keeping the said High School Diploma

inside an envelope together with my other important
documents;

4. That, sometime on November 2013, the strongest tropical

cyclone ever recorded “Super Typhoon Yolanda” devastated
our place. I tried to locate the said High School Diploma but I
could not find the same;

5. That I have exerted my best efforts to locate and recover the

aforesaid Diploma, but my efforts proved futile. I now
consider the same irretrievably lost;

6. That I am executing this affidavit to attest the veracity of the

foregoing, and for whatever legal purpose this may serves
best.

IN WITNESS WHEREOF, I have hereunto affixed my signature

this 23rd day of January 2019 at Macrohon, Southern Leyte,
