ANNUAL ACCOMPLISHMENT REPORT


F.Y 2018

I. INTRODUCTION

The 2018 Adia Elementary School Annual report articulate and


demonstrates the dynamic nature of our school, our commitment to
excellence and relentless pursuit of achieving our best. It specifically
highlighting this year’s distinct projects and programs, education outcomes,
outstanding accomplishments, financial performance and action plans which
are anchored to Deped Mission, Vision and Core Values.

II. OVERALL ACCOMPLISHMENT OF PROGRAMS AND PROJECTS

School-Based Feeding
25 out of 37 or 68% undernourished pupils gained weight
Program
Physical Fitness & Sports Procured and acquired sports equipment from MOOE
Participated in the District and Unit Meet.
School-Based Management The school has been practicing SBM with some levels of
(Levels) difficulty in implementing it. There is a need to train
teachers and principal to achieve expertise in the
practice of SBM.
Professional Development 3 out of 8 personnel were enrolled in graduate studies

WINS (Wash in Schools) Participated in the RHU activities like deworming.


Proper segregation of garbage.
Health Education/ Deworming 145 out of 164 or 88% were dewormed.
23 out 29 or 79% of Grade I pupils were vaccinated of
measles and tetanus diphtheria
8 out 11 or 73% were vaccinated of Anti Cervical cancer
Dental Health Program 100% were check-up and inspected
MOOE (Utilization of Fund) Monthly Utilization of fund were liquidated and validated.
Reading Intervention Program 5 pupils were undergo reading intervention.

VI. PROGRAMS ANG PROJECTS

1. Computerization Project
The school received complete set of computers courtesy of LGU
through SEF Funds.

2. Localization and Indigenization of Curriculum


Local stories in the barangay was used to replace foreign stories in
teaching especially in AP. Also products of the community was given as
example instead of products not known to pupils.

3. Health and Nutrition Program


The school has sustained the feeding program through SBFP,
canteen fund, MOOE and donations from private organizations and individual.

4. Mother Tongue-Based Multilingual Education


The common language in the barangay is Tagalog. It is being used
in teaching selected subjects.
