SUBJECT: Unusual incident report of MTD Ambulance with plate number IOB420

1. This pertains the unusual incident report of Ronald C. Salise official driver of FDC on
or about 061445 December 2018. The undersigned ordered Mr. Salise to drive the
MTD ambulance due to non-availability of BHBI drivers which at that time are all
deployed outside NGAC and due to exigency of the situation, As a Camp Manager I
opted to direct said driver to transport Engr. Arneil Castro who suffered a heart
attack at Terp Asia Job Site.
2. When said ambulance reach Capas Tarlac Medical Center the driver maneuvered
backward to unload Engr. Castro, he accidentally hit the hand rail at the emergency
area, which causes the denting of the rear-door of the ambulance. (See attached
picture)
3. To date, the ambulance rear-door is not functional.
4. Recommendations;
a. MTD main office to coordinate with the insurance company on what
supporting documents needed to restore the rear-door; and
b. Hire a qualified and certified ambulance driver.
