Cavite was a computer-based application that would convey all the patient dental
information. The system would help the dentist and the assistant to record easily, provide

that allows addition of new patient. The system also had Appointment module where the
user could add schedule for the patients. It also contained Billing module where the total
bill of the patient will be computed. It also catered Report module wherein the clinical
status can be checked. Moreover, there is Maintenance module that will allow the users to
update the tooth condition, discount, payment, procedure, and user account. Lastly, the
database module is where the admin can backup and restore data.

Project Structure
The project structure consisted of detailed graphical user interface (GUI) with

username and password before proceeding on main menu form. The system has two user
levels for login. First, the administrator who has access to all modules on the system and
the other one is the user who has limited access on the modules.

their password by inputting the username first before the security questions appear. If the
answer was correct, the user can already change for a new password and retrieve the
account.

created to easily store, modify, retrieve and create back-ups for the patient’s dental health
