Nursing-Sensitive Quality Indicator (NSQI) Project Description

Project Background
In February 2008, the Washington State Hospital Association (WSHA) and the
Northwest Organization of Nurse Executives (NWONE) signed a Memorandum of
Agreement to work collaboratively with nursing union representatives to consider and
recommend specific policy, program, or legislative changes related to nurse staffing and
quality of care. WSHA and NWONE agreed to encourage all hospitals to report data on
five selected nurse sensitive quality indicators. All Washington hospitals will be asked to
collect and then report on the five indicators beginning October 2008 and on a monthly
basis moving forward. It is critical that hospitals participate in the NSQI project to
maintain the momentum of collaboration and ensure any future legislative action is
based on solid data.

The Indicators
To minimize the burden of data collection for the NSQI project, hospitals were surveyed
to determine which measures they were already collecting. Based upon the results, the
steering committee overseeing the Memorandum of Agreement selected five NQF
endorsed nursing-sensitive care performance measures:
• Patient falls - All documented falls with or without injury, experienced by patients
on a unit in a calendar month.
• Patient falls with injury - All documented patient falls with an injury level of minor
or greater.
• Pressure ulcer prevalence - The total number of patients that have nosocomial
(hospital-acquired) stage II or greater pressure ulcers.
• Skill mix - Percentage of hours worked by RN, LPN, UAP and contract staff with
patient care responsibilities by type of unit.
• Nursing care hours per patient day - The number of productive hours worked by
RN nursing staff per patient day and the number of productive hours worked by
nursing staff (RN, LVN, LPN, and UAP) per patient day.

Reporting Tool
At least initially, reporting of NSQI data will be done through a WSHA developed secure
web-based software program. The data tool, which WSHA developed for capturing
quality data for performance improvement, is called the WSHA Quality Benchmarking
System. Using this tool, hospitals will be able to compare their data among units as well
as look at overall hospital information and to trend data over time. One person at each
hospital will have administrative privileges for access to the system and can grant
permission to others to enter or access data. WSHA will hold trainings on all aspects of
the indicators and the system, as explained below. WSHA will also provide on-going
technical assistance for the duration of the project.

Pressure ulcer prevalence

Description: The total number of patients that have nosocomial (hospital-acquired)


stage II or greater pressure ulcers on the day of the prevalence study.

Numerator Statement: Patients surveyed on an eligible reporting unit that has at least
one stage II or greater nosocomial pressure ulcer on the day of the prevalence study.
Denominator Statement: All patients on the selected unit at the time of the study who
are surveyed for the study by Type of Unit.

Patient falls

Description: All documented falls with or without injury, experienced by patients on an


eligible unit in a calendar month.

Numerator Statement: Number of patient falls, with or without injury to the patient, by
Type of Unit during the calendar month x 1000.
Denominator Statement: Patient days by Type of Unit during the calendar month.

Patient falls with injury

Description: All documented patient falls with an injury level of minor or greater. An
injury level of minor or greater defined as:
• Minor – results in application of a dressing, ice, cleaning of a wound, limb
elevation, or topical medication
• Moderate – results in suturing, application of steri-strips/skin glue, or splinting
• Major – results in surgery, casting, traction, or required consultation for
neurological or internal injury
• Death – results in death as a result of the fall.

Numerator Statement: Number of patient falls with an injury level of minor or greater by
Type of Unit during the calendar month x 1,000.
Denominator Statement: Patient days by Type of Unit during the calendar month.

Description:
• Percentage of hours worked by RN nursing staff (employee and contract) with
patient care responsibilities by type of unit.
• Percentage of hours worked by LVN/LPN staff (employee and contract) with

• Percentage of hours worked by UAP staff (employee and contract) with patient
care responsibilities by type of unit.

Nurse Sensitive Quality Indicator Reporting – Getting Started with the Tool

The tool used for reporting NSQI data was developed by WSHA’s Patient Safety
Program as part of its Quality Benchmarking System (QBS). The system is a
secure web-based application that allows users to input, track, compare, and
analyze quality data through a web browser. There is no charge to members for
use of the QBS system.

The tool is accessed by going to a secure website logon page and entering a
username and password. Data is uploaded to the QBS server using an Excel
spreadsheet. This data, protected by a password and stored on a secured
remote server, can then be displayed using the application.

Once the data is uploaded, the system has the ability to create trend, bar, and
control charts and allows users to make notations. In addition, these charts can
be exported for use in Word documents and PowerPoint presentations.

System Requirements

In order to most effectively use the NSQI Reporting Tool on your computer,
please make sure to:

• Use Internet Explorer version 5 or higher


• Enable cookies
• Enable Java
• Enable JavaScript
• Disable any pop-up blockers that may be in use

If you have any questions about these instructions, please contact your
Information Technology department.

In order to access the reporting system, you will need a user name and
password. Each hospital has identified a NSQI project lead that was given login
information upon enrollment in the project. If you do not know who the project
lead for your organization is, please contact Jane Feldman by e-mailing
janef@wsha.org.

Once you have a login, open your web browser and go to: (WEBSITE TO BE
SENT UPON ENROLLMENT) you will be prompted to log in. Once you are
logged in, use the directions on the following pages to view or upload your data.

Basic Navigation
In order to make navigation easier, take a look at the different sections of the
navigation screen:

Menu

The menu consists of a series of folders and sub-folders, which are opened by
clicking on them. These folders allow you to perform different tasks within the
NSQI Reporting Tool including viewing the Dashboard and Scorecard, viewing
particular trends, and uploading data. After clicking on one of these tasks, the
control frame and output screen will update and allow you to make further
selections.

Control Frame

The Control Frame allows you to make additional choices based on the task
selected in the Menu. Use the pull down menus, radio buttons and check boxes
to make your selection and then click the “Display” button. The resulting output
will appear in the output screen.

Output Screen

The Output Screen is where the data is viewed in Dashboards, Score Cards and
Indicator trends. Clicking on certain items in the Output Screen will allow for
further drilldown and additional information.
Menu Bar

The Menu Bar contains context-sensitive buttons for additional commands, like
emailing and exporting. The “exit” button is always available on the far right end
of the menu bar, and allows you to exit and sign out of the NSQI reporting
application.

The Menu
The Menu provides access to the different tasks that you can perform in the
reporting system. Use the Menu to access the following features:

Statit piMD: This will take the output screen to the default display that is seen
when first logging on.

Dash Board: The Dashboard displays a summary of all of the indicators


available for display. To view the Dashboard, select a view in the control frame
and click “Display”.

The indicators are grouped by type, and the Dashboard shows the status of each
indicator. If there are further breakdowns by group, they can be accessed using
the drilldown arrow on the right side of the dashboard. To view the Scorecard for
each indicator in the pane below the Dashboard, click on the square to the right
of the indicator status. Further instructions are listed in the pane directly below
the Dashboard.

The Scorecard: This displays a table of indicators by type. An indicator view is a


collection of indicators that are organized for viewing together. Select this in the
Control Frame, along with the Group (the type of indicator, e.g. Nursing-Sensitive
Indicators) that you would like displayed. For further instructions on how to

display data on the Scorecard, look in the output screen before making your
selection.

The status shows column shows both the current state of each indicator, whether
the indicator has reached the target for the most recent period, with a colored
circle. The colored triangle shows whether or not the indicator has improved or
deteriorated since the last period. The Current Value column displays the most
recent data that has been uploaded. The Target is a constant value, and may or
may not be used for each indicator. The SPC alert will show a colored square if
that indicator’s most recent point is statistically different than previous points.
The Updated column shows how recently the data has been uploaded for that
particular indicator. The small L symbols to the right of the table are links to
definitions of each indicator.

Find Indicators: Use this tool to view a summary of indicators that meet specific
criteria. This is helpful for finding a particular indicator within a large group of
indicators. The criteria can be selected in the control frame, and can be based
on the indicator Title, Owner, Expert, Target Status, or Validation Status. You
can also choose to view only the indicators with a current SPC Alert. For
example, a search was performed for indicators with “TESTING” in the title and
the following summary table was the result:

Uploading Data
The following instructions will explain how to upload nurse sensitive quality
indicators data to the NSQI Reporting Tool so that it can be viewed.

1) Log on to the QBS website using your username and password [insert
OneHealthPort wording].
2) Locate the blue folders along the left side of the screen, and click on the
one labeled “Upload Data”. The folder will expand, and continue by
clicking on “Upload [Your Hospital] Project Data”.
3) In the blue area in the lower left corner, select the project that you wish to
upload data for and click on the “Display” button”.
4) If you do not already have a blank project Excel file saved to your
computer, click on the “Download [Your Project] Project File” button.
When asked whether you want to open or save this file, click on the “save”
button and save the file to an easy-to-remember location.
5) Locate the downloaded Excel file on your computer and open it.
6) When the following screen pops up:

Select “Enable Macros” to continue.

You are now ready to enter data into the spreadsheet. Please have the following
data ready before completing the spreadsheet:

1) The month that data was collected


2) The units that data was collected from
3) The data that was collected for the corresponding indicator.

The data should be entered as shown in the following example:

Choices for the month and unit are selected using a pull-down menu. The units
listed will be the units that have been selected for data collection by your
organization. There will be a separate data-entry spreadsheet for each indicator,
as well as a spreadsheet that collects information on the number of Patient Days
for each unit.

After preparing the spreadsheet, return to the data upload project data screen.
This time click on “browse” and locate the saved data upload Excel file with the
newly entered information. Click on “open” and then click on the “Upload Excel
File Now” button. After doing so, the application will either show you a list of the
uploaded information, as well as a list of any previously entered data that has
been overwritten. If you receive an error code, there may be something wrong
with the formatting of the data in the upload spreadsheet. Please refer to the list
of upload errors for help in fixing any errors encountered when uploading data.

Viewing Your Data
Now that you have uploaded your data, you can view your trends by clicking on
the individual indicator names in the Scorecard. Clicking on one of these names
brings up a new window that shows the selected trend.

The trend chart is at the top of the new window, and the data table is below the
trend chart. Further down in the window are any comments specific to the
indicator.

These comments can include things like the indicator description, the process
expert, and other relevant notes.

Clicking on individual data points in the trend chart brings up more choices:

By selecting “Compare Unit Performance”, a table is displayed that shows the


values for each unit for that particular time period:

Selecting “Edit Comment / Action Plan” allows the user to enter information about
a particular chart point in a text field. This comment is then saved to the chart
and is accessible to anyone viewing the trend. The presence of a comment or
action plan is designated by a green “x” symbol on the chart.

Data Export

The Nursing-Sensitive Indicator Application is flexible on how the data is


exported to be used in other applications. The data can be sent directly to a
printer, printed to a Word document, or exported to a statistical software package
or spreadsheet application (e.g. Excel).

To print a chart, click on “Printable Version” in the top left corner of the trend
display:

You are then presented with two choices:

Click on the button to send to a printer.

Click on the button to send to a Word document. Additionally, once the data
is in Word format, the charts and tables can be cut and pasted into a PowerPoint
presentation.

Another option is to export the data to Excel or a statistical software package for
analysis. To do so, select the indicator you wish to export and the click on the
button in the upper right corner of the main screen. You can then
select the file type to export, and the location of the saved file. You can then use
the appropriate application to further analyze the data.

Upload Errors

Data file must be called: Patient_Falls_(Facility).xls


Please rename the file to meet these requirements and then upload it again.

This error is a result of the file being named incorrectly, or no file being selected
before the upload button is clicked. Make sure to select an upload file with the
name in the format:

Patient_Falls_(Facility).xls

with the title representing the indicator you are uploading, and the word Facility
replaced with the name of your facility. This will usually be a shortened version
of your facilities’ name and will be correct when the upload file is first downloaded
for data entry.

Excel file contents does not match Hospital:Facility

This error is caused when a file is uploaded that does not match the expected
facility name. Please make sure that the facility in the filename matches the
facility that you logged in under.

Please Correct and re-upload the file

You cannot enter a zero value for the number of containers or amount of product
used. If a unit does not use a particular product for a particular month, just
ignore that product and leave out that row.

If you received this error when entering data, it means that you were trying to
type in data for a unit or date that does not exist in the QBS database. If you are
sure that this unit or date is not listed on the pull down menu, please contact your
administrator.

Frequently Asked Questions

General Project Questions

Is there a charge for using the reporting system?


No. The WSHA system used for reporting the NSQI data is funded through WSHA dues.
This system was originally developed as part of the WSHA patient safety program and is
called the Quality Benchmarking System. The system was developed in response to
member requests to be able to track and share results on quality improvement projects,
such as hand hygiene.

Who sees the NSQI data?


By participating in this project, hospitals have agreed to share their data with WSHA staff
and members of the steering committee overseeing the Memorandum of Agreement
signed by WSHA, NWONE, and the three nursing unions. Each of the signatories for the
agreement is represented on the steering committee. The committee has agreed to
develop a process to ensure the data is kept confidential.

Indicator Definitions

How were the NSQI Indicators selected?


WSHA surveyed Washington hospitals to determine which indicators were most
frequently being collected already. The results of the survey were shared with the
steering committee overseeing the Memorandum of Agreement signed by WSHA,
NWONE, and the nursing unions. Based upon the results of that survey, the steering
committee selected the five Nurse Sensitive Quality Indicators for this project.

How do I get a username/password for the NSQI reporting system?


Please contact the NSQI Project lead for your hospital to obtain a username and
password. If you do not know who the lead is you can contact, (Name) at WSHA at
(phone) or (email).

Exporting Data

Can I export data for use in PowerPoint Presentations?

Performance Measure Name: Pressure Ulcer Prevalence

Description: The total number of patients that have nosocomial (hospital-acquired) stage
II or greater pressure ulcers on the day of the prevalence study.

Rationale: The incidence of hospitalized patients developing pressure ulcers has been
reported to range from 2.7 percent (Gerson, 1975) to 29.5 percent (Clarke and Kadhom,
1988). Certain circumstances (e.g., immobility, incontinence, impaired nutritional status,
critical illness, etc.) further increase the risk for selected patients. The development of
hospital acquired pressure ulcers (HAPU) places the patient at risk for other adverse
events and may lead to increased lengths of stay. HAPUs also increase resource
consumption and costs. Recommendations from the guideline Pressure Ulcers in Adults:
Prediction and Prevention (AHCPR, 1992) include the identification of individuals at
risk and early intervention with a goal of maintaining and improving tissue tolerance in
order to prevent injury. In most vulnerable patients, reducing risk factors and
implementing preventive/treatment measures will reduce the incidence of new pressure
ulcer development and prevent the worsening of existing ulcers. Nurses and nursing-care
interventions play an important role in pressure ulcer prevention and management. The
use of this prevalence measure allows organizations to monitor this important patient
outcome at points in time and examine institutional processes.

x Review and follow the Prevalence Study Methodology (see Appendix D)
x For the purposes of this measure, and to maximize reliability across organizations,
only hospital-acquired stage II or greater ulcers are included in the numerator.

Measure Analysis Suggestions: In order to further examine the issue of pressure ulcers
within your facility it may be useful to analyze the pressure ulcer rate by other important
factors. For example, it may be valuable to calculate the rate of risk assessments
completed and use of prevention protocols. To facilitate these analyses, optional data
elements have been added to the data collection tool. These include, but are not limited to
risk assessment and prevention actions. These data are not required to calculate the basic
prevalence rate but may be useful to further understanding of your organization’s
pressure ulcer prevalence and initiate quality improvement activities if appropriate.

Terminology:
Community Acquired Pressure Ulcer A community acquired pressure ulcer is
defined by:
x Ulcer discovered/documented on first day of hospitalization; or
x Prevalence study was done on day one of patient’s hospital stay and ulcer
was already present; or
x Ulcer of Stage II or greater discovered/documented on second day of
hospitalization; or
x Prevalence study was done on day two pf patient’s hospital stay and a
Stage II or greater ulcer was already present.

Hospital Acquired Pressure Ulcer (Nosocomial) Nosocomial refers to new


ulcer(s) developed after admission to a facility (also termed hospital-acquired).
All pressure ulcers not meeting the community-acquired criteria should be
designated as hospital-acquired pressure ulcers.

Pressure Ulcer Any lesion caused by unrelieved pressure resulting in damage of
underlying tissue. They may be located over bony prominences or under a
medical device/equipment. They are staged according to the extent of observable
tissue damage.

Description: All documented falls with or without injury, experienced by patients on an


eligible unit in a calendar month.

Rationale: Patient falls occurring during hospitalization can result in serious and even
potentially life threatening consequences for many patients. Efforts to reduce this adverse
event have included the development of tools to assess and identify patients at risk of
falling and the implementation of fall prevention protocols. More recently, research has
suggested that staffing on patient care units, specifically the number of professional
nurses, may impact the incidence of this patient outcome. Nurses are responsible for
identifying patients who are at risk for falls and for developing a plan of care to minimize
that risk. High performance measure rates may suggest the need to examine clinical and
organizational processes related to the identification of, and care for, patients at risk of
falling, and possibly staffing effectiveness on the unit.

Data Accuracy:
x “Fall Risk” Each facility should establish which patients are at risk based
on their particular screening process or assessment tool. For example, in
the literature, a cut-off score for the Morse scale is > 45 and < 3 for the
Schmid. However, your facility may select a different risk level to fit the
needs of your patient population. In addition, some facilities may not
require calculation of a risk score on low risk patients. In this case, the
scale score will not be available.
x Eligible reporting units for this measure are defined by the allowable
values for the data element, Type of Unit. Data collection at the specific
unit level captures data on patient outcomes and nurse staffing within a
given unit. Therefore, for the purposes of this measure, patient falls are
that occur while off the unit are not counted in the unit-level reporting.

x An eligible reporting unit will report fall data by calendar month. In
addition, each unit that reports fall data, must also collect patient day data
for the same month (as outlined in the data element, Patient Days – also
see Appendix F: Table 3.1 Patient Day Reporting Methods) in order to
calculate fall rates.
x Fall rate is calculated by multiplying the numerator by 1,000 and then
dividing by the denominator.

Measure Analysis Suggestions: In order to further examine the issue of falls within your
facility it may be useful to calculate the number of patients who were assessed, who were
at risk and what their risk level was. It may also be useful to identify patient falls that
involved staff intervention. To facilitate these analyses, additional data elements could be
collected that are not required for calculating the primary measure rate. These data
elements are listed below and may be useful in further analysis.

Terminology:
Assisted Fall A fall in which any staff member (whether nursing service
employee or not) was with the patient and attempted to minimize the impact of
the fall by easing the patient’s descent to the floor or in some manner attempting
to break the patient’s fall. “Assisting” the patient back into bed or chair after a fall
is not an assisted fall. A fall that is reported to have been assisted by a family
member or visitor also does not count as an assisted fall.

Fall An unplanned descent to the floor (or extension of the floor, e.g., trash can or
other equipment) with or without injury to the patient.

Fall Risk Assessment A formal evaluation of a patient’s likelihood to experience


a fall. Several assessment instruments are available in the literature. Two
commonly used instruments are the Morse and Schmid Scales. Organizations
often adapt or create different scales depending on population needs.

Fall Risk A defined level at which patients are determined to be “at risk” of
experiencing a fall. For example, in the literature a cut-off score for the Morse
scale is > 45 and 3 for the Schmid. However, an organization may select a
different risk level to fit the needs of their patients and may not require a
calculation of a risk score on low risk patients

Fall Injury Level The extent of injury experienced by a patient following a fall.
National standards for injury levels have yet to be established; therefore the
following designations are used for the purposes of the measures set.
x None – patient had no injuries resulting from the fall
x Minor – resulted in application of a dressing, ice, cleaning of a wound,
limb elevation, or topical medication
x Moderate – resulted in suturing, application of steri-strips/skin glue, or
splinting
x Major - resulted in surgery, casting, traction, or required consultation for

Rationale: Nursing care hours per patient day measures the supply of nursing relative to
the patient workload. The relationship of nurse staffing to the quality of patient care and
patient outcomes has been the subject of multiple research studies in recent years. The
total number of nursing care hours per patient day reflects time constraints on nursing
staff that can constrain quality of care, resulting in nurses being stressed, fatigued or
distracted, increasing the risk for mistakes or omissions in care. Examining the
relationship between nursing care hours, and processes and outcomes of care within
health care organizations, may identify opportunities to improve care delivery, patient
outcomes, and provide an evidence base for determining the most effective staffing
levels.
