PETITION

PETITIONER in the above-entitled case, by himself, and

unto this Honorable Court, most respectfully states:

1. That he has passed the 2011 Bar Examinations and

has been since then, and up to the present, a member in good
standing of the Integrated Bar of the Philippines and of the
Philippine Bar;
2. That he is a Filipino, 28 years old having been born on
January 1, 1991, married and a legal resident of Salvador,
Cortes, Bohol with Telephone No. (032) 256-9013;
3. That he is a member of the Integrated Bar of the
Philippines, Bohol Chapter in good standing with Roll No.
12345;
4. That he has been a resident of the Philippines since
birth and maintains a regular place of work or office here in
the City of Cebu for the last six (12) years;
5. That he has been appointed and commissioned as
Notary Public for the City of Cebu last 2016 and said
commission had expired last December 31, 2018;

6. That he desires that said commission be again

renewed for another period of two (2) years up to the 31 st day
of December 2020, and that he has all the qualifications and
none of the disqualifications to the Office of Notary Public;
7. That he has paid the necessary Professional Tax as
lawyer for the year 2019 under Professional Tax Receipt (PTR)
No. 5644965, dated January 31, 2019, issued at Cebu City,
and had likewise paid his IBP dues for Fiscal Year 2019 as
evidenced by an Official Receipt bearing the Number 711053
issued by the IBP-Bohol Chapter on January 31, 2019.
Machine copies of which are hereto enclosed and marked
respectively as Annexes “A” and “B” and made integral part
of this petition;
8. That he has not been convicted nor charged of any
crime involving moral turpitude;
9. That he is submitting the specimen of his signature
and facsimiles of his notarial seal on a separated paper hereto
attached and marked as Annex “C” and made integral part of
this petition;
10. That he is likewise submitting a certification of good
moral character by two (2) executive officers of the local
chapter of the Integrated Bar of the Philippines in this place
where he is applying for renewal of his notarial commission
hereto enclosed and marked as Annex “D” and made integral
part hereof;
11. That furthermore enclosed hereto are three (3)
passport size color photographs of the herein petitioner taken
last January 1, 2019 with signatures appearing at the bottom
part of said photographs hereto marked as Annexes “E”, “E-
1” and “E-2”.

WHEREFORE, it is most respectfully prayed that

petitioner’s commission as Notary Public be please renewed in
his name, and that petitioner further prays for other reliefs
and remedies just and equitable under the circumstances.

the City of Cebu after being sworn to in accordance with law,
hereby depose and say: that I am the petitioner in this case;
That I have caused the preparation of the foregoing Petition
and have read and understood the same which are all true
and correct to the best of my knowledge, belief and available
records; That I certify that I have not commenced any other
action or proceeding involving the same issues and the same
parties before the Supreme Court, Court of Appeals, or any
other tribunal, agency or body, and to the best of my
knowledge, no such action or proceeding is pending but if I
should learn thereafter of a similar action that has been filed, I
undertake to report the same to this Honorable Commission
pursuant to Adm. Circular No. 04-94 in relation to Revised
Circular No. 28-91.
