Purpose of Document

The purpose of this document is to make sure that what task we have already
done in CCM Health application and how the application features were
behaving previously and what amendments we made after that and listed down
in this document with screenshots.

1: Assign Counselors in Bulk. It was a tier 2 task and on admin total patient section previously
when we try to assign a Counselors in a bulk we had no option to assign it in a bulk form.
Now we have provided a checkbox functionality where we can select a multiple record via
checkboxes for assigning them a counselor in a bulk. How we can find this bulk section please
follow the steps.
 Go to CCM Health application and login by providing credentials.
 After login just click on left menu on dashboard admin total patient. And you will be
redirected to admin total patient list where you can select multiple check boxes for
assigning Counselors in a bulk please see screenshot

2: When we go to total patient section and assigned a counselor to a patient and after saving it does
not refresh records. It should automatically refresh the records.

This was a (Tier 1) task as well. Previously when we were assigning a counselor and saving a record after
assigning counselor we are refreshing a page manually. Page was not refreshing automatically.

Now in current version of the application developer fixed the issue and refreshed the page automatically
that assigned option could display in assigned section.

3: PDF was not generating to the latest version in final care plan section. It was a Tier 1 task and in
final care plan section PDF file was not generating.

Now in new release developer fixed that issue and now user will be able to generate a PDF file in final
care plan section. Please see screenshot

4: Make the Search 2 Steps in every Table Column Selection to Search in Data-Base. This task was
related to (Tier 1).

Previously there was not an efficient search in every table column. But in current release developer
make an efficient search where user can make a search for every single column. For performing this
search need to follow the steps below.

Go to CCM Dashboard > Click in left menu Admin total patient > it will redirect a user in a detail section >
And now user can search and find records. Please see screenshot below as well.

5: CCM Enrollment Status Two Statuses has to be Fixed and Merged and Assigned Properly as in SQL
its CCMSTATUS and ENROLLMENTSTATUS.

Previously there were no two statuses were merged and assigned properly. But now in a recent fixes
developer fixed this issue also displayed these two statuses in total patient detail section. Please see
screenshot below.

6: Fix CCM Patients Table and Convert it with AJAX. This task was in (Tier 1).

Previously in this section patient table speed was not improved / slow. But now in recent fixes patient
table speed has been improved like admin total patient table. Please see screenshot below

7: Rename CCM Patients to Active Work Que.This task was in (Tier 1).

Previously in a left menu section there was a name displaying CCM patient. But in recent release
developer renamed this from CCM patient to Active work log. Please see screenshot below.

8: First name last name and date of birth need to make editable on patient details. This task related to
(Tier1).

Previously on patient detail section first name, last name and date of birth was not editable. Now after
discussion developer made these options editable in new fixes. Please follow the steps for this fix.

Go to admin total patient > Detail > Contact Pref > Basic info and you will found the fields editable

Please take a look in screenshot.

9: Bring Additional Column in CCM Patient Date & Time Stamp of Assigned (Should be Sorted) and
Date & Time Stamp of Enrolled (Should be Sorted). This task was in (Tier1).

Previously there was no Date and time stamp in CCM patient section now in latest fixes developer added
these two columns in CCM patient section.

Go to dashboard > Click on admin total patient > and right scroll and you will see the changes. Please see

10: Re Assign All statuses to SQL by making them not enrolled and then re assign. This task was in Tier
1 section.

In this task developer made an amendment in DATABASE and reassign all structures in DATABASE
Reassign all the roles in SQL starting from no enrolled status.

We are unable to get any screenshot regarding backend for this task.

11: In patient detail put insurance details for read only. This task was in tier 1 section.

Previously there was no readable insurance info in this section and after discussion developer displayed
this specific information in patient detail section.

On dashboard click on admin total patient > then click on detail section > now scroll down and you will
see insurance info section readable only. Please see screenshot below

12: Medicare, Medicaid and other insurances information need to make editable and keep on
insurance section. This task was in (tier 1).

Previously in insurance section Medicaid, Medicare and other insurances were not editable. But in
recent discussion and new fixes developer made insurance section editable.

Just go to admin total patient > click on details > click on contact preferences and select insurance
option after selecting this option you will see Medicaid, Medicare and other insurances option will be
editable in insurance section. Please have a look on screenshot.
