4. Notice and Certificate of Posting

5. Certified machine copy of the Official Receipt of the filing fee
6. Clearance from:
a. employer, if employed
b. the National Bureau of Investigation
c. Philippine National Police
7. Proof of Publication.
8. An affidavit of publication from the publisher
9. copy of the newspaper clippings
