AFFIDAVIT

I, PO3 ELENITA HERNANDEZ., Filipino, of legal age, an active personnel of the

Philippine National Police, Pasig Police District, stationed at 102 C. Raymundo Ave, Pasig,
Metro Manila, after having duly sworn to in accordance with law, hereby state and depose that:

1. On 16 December 2017, at around 1:30am I was assigned at the women’s helpdesk of our
headquarters

2. That on or about such date and time, PO3 Pedro Silva referred to me a woman, whom he
failed to identify, and a taxi driver by the name of Juan Miguel Dela Peña

3. That I learned from the taxi driver that the said woman boarded his taxi on December 15, 2017
at around 10:45pm

4. The woman is said to have been carrying a luggage and was accompanied by a man who helped
her get a taxi

5. Mr. Dela Pena said that initially the man who helped the woman get a taxi told him to bring the
woman to SM megamall, however while on their way, the woman said to take her instead to the
Pasig cemetery

6. Mr. Dela Pena told me that what prompted her to bring the woman to the police station
was that the woman was unresponsive to his questions and was making strange acts
like singing the alphabet song

7. Fearing that he might not get paid after taking the woman to the said destination by
reason of the latter’s unresponsiveness and strange acts, he decided to take the woman
to the nearest police station

8. That when I (PO3 Elenita Hernandez) asked the woman as to her name, said woman did
not respond

9. That I personally observed that the woman was giving a blank stare to anyone who
would approach her

10. That I offered food to her but she did not touch or eat the food I offered
11. That around 4:00 am, and after I already allowed the taxi driver to leave, said woman
was still unresponsive to the questions I ask her and continue to just give a blank stare

12. That I inspected her luggage which was partially open, and I noticed that it contained
plates, pot, and other kitchenware.

13. However I was unable to get any identification cards

14. At that time, it appears to me that she was not in her proper composure, and may
possibly be mentally sick, especially since the driver said that she was singing the
alphabet and acted very weird when she was in the car

15. That because of these circumstances, I decided to call NCHM so that she can be
examined by doctors there

16. That later, around 4:40am, I personally took the woman to Mandaluyong Mental
Hospital

17. That in Mandaluyong Mental Hospital, I was directed by the guard to go to the Emergency
Room, where I was assisted by a nurse named Rachel Cartin

18. Said nurse told me that there is a doctor on duty, but the doctor was just having his mealtime
and will return after around 45 minutes

19. That I asked the nurse if it is possible for me to leave the woman in their custody even if the
doctor is not yet there as I would have to return to the headquarters immediately

20. Nurse agreed and thereafter asked me the name of the woman and other circumstances

21. I told the nurse that we failed to get the name of the woman as there are no identification cards
in her possession, and that the woman was delivered to us by a taxi driver.

22. I also told the nurse that the woman was unresponsive and was just giving a blank stare to
anyone

23. I told the nurse that we will be conducting a follow up investigation in the morning to ascertain
the identity of the woman.

24. I gave the nurse my contact number and asked her if she can update me on the status of the
woman by tomorrow

25. I left the Mandaluyong Mental Hospital around 6:00 am that day.
