JOINT AFFIDAVIT

WE, GERRY G. PABAYOS, of legal age, married, Filipino and with residence and postal
address at, Bgy. Magatas, Sibulan, Negros Oriental, Philippines, and ASTERIA C. NILLAS, of
legal age, married, Filipino and with residence and postal address at Bgy. Magatas, Sibulan,
Negros Oriental, after having been sworn to in accordance with law, do hereby depose and say:

That we both personally know the person of ARMANDO CALINTIG AGUILAR who is
the son of the spouses, MARGARITA CALINTIG and ISMAEL AGUILAR, as we are close
associates and friends of the family;

That only recently, we were shown a copy of the CERTIFICATE OF LIVE BIRTH of
ARMANDO CALINTIG AGUILAR wherein the middle name of his mother was erroneously
entered as “CALINTE” instead of “CALINTIG” which is the true and correct spelling of his
middle name;

That, we can both unequivocally state and manifest that “ARMANDO CALINTE
AGUILAR” and “ARMANDO CALINTIG AGUILAR” refers to one and the same person who is
the son of the spouses Margarita Calintig and Ismael Aguilar;

That we both know these facts to be true and correct because we are close associates
and friends of the said “ARMANDO CALINTIG AGUILAR”, and his family;

That we are executing this affidavit without mental reservation to attest to the veracity
of the above-stated facts and for whatever legal purpose this may serve him best.

IN WITNESS WHEREOF, we hereunto affix our signatures this 12th day of December,
2014 at Sibulan, Negros Oriental, Philippines.
