 II. OFFER
The testimony of the affiant ARYA STARK is being offered to
attest to the truth and veracity of the foregoing statement:
1. To file a formal complaint of QUALIFIED THEFT against
RAMSAY BOLTON and for any legal purposes it may serve.

III. JUDICIAL AFFIDAVIT PROPER

I, ARYA STARK, 29 years old, single, Filipino and residing in

Surigao City, under oath, depose that the taking of my
Judicial Affidavit was administered and supervised by ATTY.
KLAIRE CIARRA G. SANGCO and I am fully aware and
cognizant of the questions asked of me by Atty. Sangco and
my answers thereto are under oath being aware that I may
face criminal liability for false testimony or perjury. I likewise
requested that the questions asked of me be in English as well
as my answers thereto also in English which I speak and
understand for clarity and that the other information and/or
requirements of Sec. 3, A.M. 12-8-8- SC as explained to me by
the administering lawyer are contained in my Judicial Affidavit
the rest which reads, to wit:

Q1: Please state your name and other personal

circumstances?
A1: I am Arya Stark, single, a businesswoman, owner of
ABC Gadgets Merchandising.
Q2: Why are you here?
A2: To give my sworn statement by way of a judicial
affidavit, the same to constitute as my direct
testimony, in the above-captioned criminal case.
Q3: You said that you are an owner of a store, where is
it located?
A3: It is in Public Market, Surigao City, Ma’am.
Q4: Is there any event in your store that is related to
your testimony?
A4: Yes, Ma’am.

Q5: How is this related?
A5: The salesman in my store has unlawfully and
willfully taken some of my products.
Q6: Who is this salesman you are referring to?
A6: Ramsay Bolton, of legal age, a resident of Brgy.
Borromeo, Surigao City.
Q7: And when did this happen?
A7: Around 6:30 in the evening of March 4, 2019.
Q8: What happen at around 6:00 in the evening on
March 4, 2019?
A8: I entrusted the key to my salesman RAMSAY
BOLTON, with confidence asking him to lock and
secure the gadgets in the establishment.

Q9: What did you do after giving Mr. Ramsay Bolton the
key?
A9: At around 8 o’clock that evening, I called RAMSAY
BOLTON to make sure he has already locked the
establishment but he did not answer his phone.

Q10: What did you do then?

A10: On March 5, 2019, at around 7 o’clock in the
morning, I went to the establishment and was
surprised because my salesman RAMSAY BOLTON
had not yet arrived.

Q11: Did you wait for him to arrive?

A11: Yes. When I was about to call him, the saleslady of

the establishment nearby passed by so I asked her
if she had seen RAMSAY BOLTON. She said she
had not seen him that morning but had seen him
last night. He was carrying a back pack and he was
rushing. He rode a motor cab saying “UNA SA KO,
DALHON KO NI SA MGA NAG- ORDER.” (Affidavit of
witness, Ms. CERSEI LANNISTER is hereto attached as
“ANNEX A)”;

Q12: Did you ask him to deliver some gadgets to the
customers?

A12: No. I never asked him to deliver gadgets to our

customers.

Q13: What did you do then?

A13: After conversing with the saleslady, using the

duplicate key, I opened my establishment.

Q14: What happen when you open your store?

A14: I noticed that valuable items were missing: three

FUJITSU life book with serial numbers: 512093,
51209 and 502108 respectively, all valued at P80,
000.00; 2 64GB APPLE IPHONE 5 all valued at
P35, 000 and 2 Nikon D750 24.3 Megapixel
Camera all valued at P40, 000 with the total
amount of P150, 000, Philippine currency. After
discovering that the items are missing, I called
RAMSAY BOLTON.

Q15: Were you able to reach him?

A15: No, ma’am.

Q16: How can you say that it is Ramsay Bolton who took
the said products?

A16: He was alone in the establishment and the key was

entrusted to him, he had free access to the items
which he carried away.

Q17: What evidence/s can you present to prove this fact?

A17: The Police identified him through the CCTV Camera

installed in my store, Ma’am.
Q18: How did the police know of the CCTV camera?
A18: After what happened to my store, I immediately,
went to the Police Station of Surigao to report the
matter, as evidence in the Police Report.

 IV. SWORN ATTESTATION OF THE LAWYER WHO
CONDUCTED OR SUPERVISED THE
EXAMINATION OF THE WITNESS.

The undersigned ATTY. KLAIRE CIARRA G. SANGCO, of

legal age, single, and with law office address at Coreto Bldg.,
Taruc Compound, Surigao City, under oath, deposes and
states:

1. She is the Legal Counsel for the plaintiffs in the above-

entitled case;
2. She faithfully recorded or caused to be recorded the
questions he asked and the corresponding answers that the
above-named affiant gave;
3. Neither she nor any other person then present or
assisting him coached the witness regarding the latter's
answers; and
4. He conducted the examination of the witness at her law
office located at Coreto Bldg., Taruc Compound, Muntinlupa
City, Philippines.
